Job summary

Hawthorn Medical Centre is a over 6000 patient practice who are looking for a new Practice Nurse. We are a well-established training practice, with high QOF outcomes and Good at CQC.

We are looking for an experienced Practice Nurse to join our friendly team as we look to better serve the needs of our community. You will be supporting patients to be healthy, by monitoring long-term conditions and by focusing on health prevention and screening.

You will be working with a team of GPs, nurses, a Focused Care worker, and a brilliant admin team that keep the show on the road. There will be professional development opportunities, including cross-company meetings with other practice nurses and opportunities for further training.

Main duties of the job

The post holder will be responsible for the delivery of Practice Nursing services, alongside another experienced Practice Nurse, to the practice population. Supported by the Practice team, they will deliver care and focus on supporting patients to be healthy, monitor long-term conditions, health prevention and screening activities.

They will work collaboratively with the General Practice team to meet the needs of the patients, supporting the delivery of policy and procedures.

The hours of work for this post is 20 - 22.5 hours per week, flexible Monday to Friday between 8:00 and 18:30.

Assess, plan, develop, implement and evaluate programmes to promote health and well-being, and prevent adverse effects on health and well-being

Implement and evaluate individual treatment plans for patients with a known long- term condition or at risk of a long term condition

Support patients to adopt health promotion strategies that promote patients to live healthily

Assess and care for patients presenting with uncomplicated wounds
