Job summary

This is a new ANP role that allows the successful candidate to work across Hawthorm Medical Centre- Walk-in Service. There is flexibility about which days or hours you work. Our core hours are Monday-Friday 8am-8pm and Saturday and Sunday 10am-5pm. The successful candidate's timetable can be flexibly organised around their needs in consideration with the needs of the population at Hawthorn.

Main duties of the job

The post holder will demonstrate courageous and critical thinking and is a caring, compassionate and committed experienced nurse who, acting within their professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care. They will demonstrate safe, competent clinical decision-making and expert care, including assessment and diagnostic skills, for patients .

The post holder will commit to demonstrating critical thinking in the safe clinical decision-making process. They will communicate and work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership as required.

About us

Hawthorn Medical Centre has a registered list of 6000+ patients and an unregistered walk-in element that sees 15000 patients a year.

We are looking for ANPs to join our established team of ANPs and GPs that run the walk-in service. Each day is different, so you need to have great diagnosis skills and be able to think on your feet. You will be part of a team that provides support, training, and a good amount of tea and cake! You will also be joining the wider team of ANPs across the company who get together annually for learning and development opportunities.

Job responsibilities

Communication

Communicate effectively with patients and carers, recognising the need for alternative methods of communication to overcome different levels of understanding, cultural background and preferred ways of communication

Utilise communication skills to support patients to adhere to prescribed treatment regimens

Delivering a quality service

Recognise and work within own competence and professional code of conduct as regulated by the Nursing and Midwifery Council (NMC)

Produce accurate, contemporaneous and complete records of patient consultation, consistent with legislation, policies and procedures

Deliver care according to the NSF and the National Institute for Clinical Excellence (NICE) guidelines and evidence-based care

Utilise the audit cycle as a means of evaluating the quality of the work of self and the team, implementing improvements where required

Participate in the management, review and identify learning from patient complaints, clinical incidents and near-miss events utilising a structured framework (eg root-cause analysis)

Team Working

Work as an effective and responsible team member, supporting others and exploring the mechanisms to develop new ways of working

Delegate clearly and appropriately, adopting the principles of safe practice and assessment of competence of those taking on delegated duties

Management of Risk

Manage and assess risk within the areas of responsibility, ensuring adequate measures are in place to protect staff and patients

Monitor work areas and practices to ensure they are safe and free from hazards and conform to health, safety and security legislation, policies, procedures and guidelines

Ensure safe storage, rotation and disposal of vaccines and drugs is undertaken.

Undertake mandatory and statutory training

Utilising information

Use technology as an aid to management in planning, implementation and monitoring, presenting and communicating information

Review and process data using accurate Read codes about patients in order to ensure easy and accurate retrieval for monitoring and audit processes

Learning and Development

Act as mentor to students, assessing competence against set standards as requested and if appropriately qualified

Make effective use of learning opportunities within and outside the workplace, evaluating their effectiveness and feeding back relevant information
