Job summary

Patient Navigator, based at Glodwick Medical Practice (Jarvis and Sun Valley Medical Practices), Oldham

WTE 37.5 hours per week // NHS Pension // 35 days annual leave pro rata Part time considered.

Main duties of the job

The ideal candidate will have a positive and calm manner, be efficient and be good at multi-tasking and prioritising. They will be able to work systematically and be able to follow procedures and protocols closely. They will be good at IT and at making sense of complicated systems. They will need to be able to work in a busy environment, being helpful and efficient when communicating with other staff and patients. Experience in a similar role is preferble.

Job description

Job responsibilities

As a Patient Navigator, you will work as part of the frontline patient support team. You will complete daily, weekly, and monthly tasks and will multitask and prioritise to get this work done. You will support clinical staff and problem solve for patients and display exceptional communication skills. You will be the first person the patient sees, you will be the face of the practice.

Job Responsibilities.

To greet and signpost patients to the appropriate service/clinician.

To scan and file all correspondence into patients medical records using the docman system.

To triage all clinical correspondence and workflow necessary documents to clinicians.

code all relevant clinical information into patients medical records.

Summarise all new patients medical records into the emis system as per the practice policy.

To make, cancel and amend appointments.

To follow an effective recall system for patients to include QOF recalls, childhood immunisations, smears and flu vaccinations.

To process all insurance and solicitor reports in a timely manner.

To process copies of medical requests for patients and insurance companies in a timely manner.

To complete all tasks in workflow manager to include deductions, GP2GP etc.

To efficiently deal with tasks from GPs, nurses and other staff.

To maintain the practice GSF register and the Safeguarding register.

To attend and partake in practice meetings as required.

Work safely at all times in accordance with Legislative requirements and Practice Policy and Procedures.

To promote the ethos and vision of Hope Citadel Healthcare within the practice include staff, visitors and patients; through the provision of quality whole person healthcare.

To attend and participate in any meetings required by the company, including practice meetings, and any applicable training.
