Job Title: Registered Nursing Associate (RNDA) Spinal Point Range: 10,11,12 Hours: 37.5 hours per week Department: In-patient Unit Location: St Michaels Hospice, Bartestree Reports to: Senior Nursing Team Job Purpose: To monitor, contribute and inform high-quality palliative and end of life care to patients and families. Responsibility Areas: It is expected the Nursing Associates will be able to carry out all, of the core competencies outlined in the job description but their duties and competencies will vary depending on area of work and speciality agreed extended competencies. Deliver high quality, compassionate care under the direction of a Registered Nurse with a focus on promoting patient health and independence. Work as part of a designated clinical care team delivering care that focuses on the direct needs of the patient throughout their lifespan. Clinical responsibilities Individuals care The following list is indicative as tasks and responsibilities will vary depending on the care setting: Abide by the principles of the NMC code of conduct and work within clearly defined accountability frameworks, always recognising the boundaries to the remit of the Nursing Associate role. Develop understanding of all elements of the nursing process by guided participation in the on-going assessment, planning, management and evaluation of care with increasing knowledge skills & competence. Support individuals with all aspects of care including personal care and promoting wellbeing, activities of daily living, providing person-centred care and promoting health and independence. Working as part of the multidisciplinary team through awareness raising and care navigation to other professionals as appropriate 2 JE verified: DATE Updated: DATE Measure, monitor and record observations as required TPR and BP, Oxygen saturation, peak flow, ECG, urine tests, blood glucose monitoring, fluid intake. Administer medication in a safe and timely manner as prescribed, in accordance with the organisations medicine Management policies and training. Demonstrate the ability to recognise changing priorities seeking advice and guidance from the Registered Nurse or other registered care professionals as appropriate. Report back and share information with the registered nurses on the condition, behaviour, activity and responses of individuals Contribute to the implementation of appropriate action to meet the specific physical, emotional and psychological, social, cultural and spiritual needs of individuals and carers Contribute to the delivery of complex care as prescribed by the registered nurse Contribute to the implementation and monitoring of clinical standards and outcomes Demonstrate good understanding of principles of consent and ensure valid consent is obtained prior to undertaking nursing and care procedures Demonstrate good understanding of the Mental Capacity Act / Deprivation of Liberties and applies principles to everyday practice seeking advice / guidance from the Registered Nurse or registered care professional as required Care Provision Contribute to the comprehensive holistic assessment, planning implementation and evaluation of the needs of patients on each shift. Deliver generalist nursing care interventions that is responsive to the individual patients requirements. Report any concerns or changes in patients condition and or familys wellbeing to wider community teams as appropriate. Contribute to the patient centred assessment and care planning ensuring timely review and update utilising the OACC suite of outcome measures and contribute to care plan modifications as required. Identify pre and post bereavement needs and potential issues referring on to supportive care team. Ensure that patients/carers needs are identified and refer to supportive care if needs are greater. To support the Senior HCAs when undertaking any joint visits; sharing knowledge and skills. To act as a link nurse in selected specialism e.g. diabetes. To follow all agreed hospice and community NHS clinical procedures ensuring alignment with NMC/statutory regulations related to Medicine management. To follow all agreed policies for manual handling, infection control, health and safety, ensuring a safe environment for patients, visitors and staff visiting and within the service. 3 JE verified: DATE Updated: DATE Planning Manage and prioritise own workload according to patient and family need. Undertake risk assessments, document these and implement risk reducing measures escalating to senior colleagues as appropriate. To contribute to timely patient discharge from admission. Communication Communicate and liaise with relatives and carers regarding patient status, understanding the sensitivities and complexities of each situation. Provide accurate written and verbal holistic reports in line with the Hospice and NMC guidance to colleagues during handover on patient condition, including any concerns or changes. Encourage patients/relatives/carers to express what is important to them, to identify their needs, to ensure that this information is recorded and informs the care plan. Promote collaborative working and effective communication between all members of the multidisciplinary team. Ensure the nursing voice is represented in ethical decision making. Recognise own limitations when offering emotional support. Respond to complaints in line with Hospice policy, escalating as appropriate. Promote awareness of child and adult protection issues policy and guidelines. Recognising issues relating to safeguarding vulnerable children and adults and report any problems or raise concerns to the appropriate registered care professionals Professional Leadership Act as a role model in all aspects of clinical and non-clinical nursing practice. Provide volunteers with information required to enable them to give appropriate care to patients and their visitors. Develop skills in relation to coaching/teaching individuals/carers/other staff and engage in formal and informal clinical supervision, support and mentorship. Assist in the supervision and training of health care assistants, student nurses and trainee nursing associates To deliver informal and formal training sessions for staff and others Quality, Improvement, Audit and Research. To contribute to audits and evaluation of work to develop and improve palliative/end of life care for patients and their carers. Facilitates and participates in reflective reviews of palliative care. Ensure clinical incidents /near misses are recorded and managed in a timely manner and take action to minimise risk. 4 JE verified: DATE Updated: DATE Health and Safety Act at all times to promote the safety and wellbeing of patients, visitors and volunteers. Ensure the environment is free from hazards and all nursing equipment is clean, in good working order, safely and appropriately used and stored. Assist in maintaining a clean environment that is conducive to safe practice. Responsible for the timely disposal and documentation of clinical waste and Laundry. Safe storage and use of hazardous substances according to COSHH regulations. Checking building, ensuring that environment is clean and tidy. Be aware of own limitations and when to seek advice and help. Understand and adhere to the Infection control practices in the community and on the Inpatient unit. Understand and adhere to the safe storage of medicines and hazardous substances policy and processes. Education Maintain and develop clinical competence through continual professional development, mandatory and service specific training, peer supervision, reflective practice and clinical supervision. Complete practice supervisor and clinical supervisor training. Maintain a professional portfolio and reflective diary.
