Job summary

The post holder is responsible for the leadership, management and development of the Hospice Community Services in Greenwich and Bexley, under the supervision of the Director of Care & Service Transformation. This will be in line with the new overarching Hospice Strategy, which will be underpinned by the Service Transformation Strategy. This is being designed to plan strategically for the growing needs of patients, particularly in the community setting, taking into account the local, regional and national policy and guidance direction and how we as a service meet those needs.

The post holder will provide management and leadership support to;

  • The team of Community Clinical Nurse Specialists (CNS) and Associate CNSs
  • The Nurses and Carers that make up our Hospice at Home Team
  • Hospice Social Work & Bereavement Support Team
  • The Hospice Chaplain

Whilst the above list specifies the direct service responsibilities of this role, it is imperative to recognise the significance of key relationships across the Hospice (see relationships diagrams at end of document), as well as the collective shared responsibility to meet the needs across all clinical services.

The post holder will provide specialist palliative care advice to internal and external professional colleagues, stakeholders, as well as patients, their families and carers.

Main duties of the job

  • To provide management and leadership support to the team of Community CNSs and Associate CNSs, the social work team, the bereavement support team and the Nurses and Carers that make up the Hospice at Home as well as the Hospice Chaplain.
  • Participating in the deployment of the Hospices over-arching strategy and all underpinning strategies.
  • Responsible to meet the strategic and governance needs within the community services as well as the working relationships required to unite and deliver these.
  • To ensure compliance with the Hospices values
  • Create and maintain seamless and effective integrated multi-disciplinary team working across all localities and teams.
  • Ability to directly manage situations among service users, families and/or staff i.e. complaints actions and learning, safeguarding, staff disciplines, capabilities or grievance procedures.
  • Develop a self-coaching team environment, oriented to trust, open communication, creative thinking, and cohesive team work.
  • To attend Hospice management meetings and relevant governance meetings
  • To liaise with external organisations as appropriate and provide information on the range of services available to patients and carers and to receive feedback on the way services are delivered.

About us

Greenwich & Bexley Community Hospice is a local charity dedicated to providing free, high-quality, compassionate care and support for people with all terminal illnesses, their families, and their carers in the London Boroughs of Royal Greenwich and Bexley. The Hospice cares for around 3,000 people each year at home, in care homes, in the local prisons, in our Hospice beds, in our Hospice-based clinics and outpatient services, and in hospital at Queen Elizabeth Hospital, Woolwich.

We employ approximately 180 staff, supported by around 400 volunteers. Our clinical team works in partnership with other providers across the Local Care Networks to deliver high quality, responsive care. During the pandemic the need for our community support has grown and we have expanded the team, introduced new ways of working to extend our reach and adapt to growing need.
