Job responsibilities
Clinical
Practice
- Maintains own clinical competence, professional
standards and ensures a model of care, which promotes excellence while
recognising the unique needs of each patient.
- Provide a high standard of holistic and
individualised, evidence based care to patients and families using Hospice
services.
- Provide expertise in symptom control and management
to patients, carers, peers and partners.
- Provide information and advice to patients, carers,
health care professionals about Hospice/ palliative care.
- Be
familiar with and adhere to Hospice child/adult safeguarding procedures and
guidelines, in conjunction with local Safeguarding Children/Vulnerable Adults
Board procedures and supporting policies/guidelines.
- Work with the Director of Care & Service
Transformation, Nurse Consultant and Clinical Leads to support and develop Hospice
staff as required.
- Set and monitor standards and outcomes of care on an
ongoing basis.
- Work collaboratively across geographical boundaries
to improve services for patients and families.
- Promote and maintain effective liaison, communication
and relationships with all of the Hospice multi-disciplinary team and external
services to ensure effective collaboration.
Leadership &
Management- To adhere to and promote the Hospice values.
- Act as an ambassador for the Hospice and palliative
care ensuring team members do this in all interactions with patients, families
and professionals.
- Through practice modelling, teaching, supervision
and management, develop staff to ensure that they are competent to meet their
job description and role, as well as enhancing their professional development
in line with Hospice policy.
- Develop a coaching and compassionate leadership
culture within the team and wider organisation by setting the example and
developing others to do the same themselves and with their respective teams /
direct reports.
- Monitor and evaluate service provision and make
appropriate changes as required.
- Manage, appraise and set objectives with direct reports, developing their full
potential, including providing support for individual learning plans and
ensuring mandatory training and clinical supervision requirements are met.
- Link team and individual objectives to the Hospice
strategy and subsequent underpinning strategies.
- In conjunction with HR department, manage staff where
there are issues of poor performance through the appropriate Hospice policies
and procedures.
- Work with HR and the Director of Care & Service
Transformation to ensure efficient and appropriate workforce planning and
recruitment.
- Ensure that accurate statistical information is collected
by team members and use this information to assess progress and improve the
quality and productivity of services under area of responsibility.
- Work with the Director of Care & Service
Transformation to initiate proposals and business cases for service development.
- Lead,
develop and support multi-agency projects within the organisation.
- Work with Hospice colleagues to ensure when and where
change is necessary, it is effectively managed.
- Develop, review and implement policies, in line with
clinical changes and statutory requirements, such as CQC, NICE Guidelines, HSE and
other local, regional and national guidance.
- Ensure adherence to Hospice policy and procedures and
that statutory regulations are met.
- As necessary and when requested, deputise for the
Director of Care & Service Transformation on clinical and management issues.
- Monitor service expenditure and to be responsible for
own budget.
- Carry out risk assessments as appropriate for the
teams and/or individuals.
Communication
- Ensure proper maintenance of confidential records on
all areas of responsibility.
- Promote and practicing excellent and complex
communication skills with patients, families and members of the
multidisciplinary team.
- Work with colleagues to ensure effective
communication channels and relationships exist within the teams, including
chairing meetings.
- Arrange regular 1:1 sessions with direct reports,
keeping an accurate note of what is discussed within them and agreed actions.
- Contribute positively to the development of the Hospice
as an anti-discriminatory organisation, actively promoting equality, diversity
and inclusion.
- Participate in locality, cancer network and lead clinical
meetings as appropriate and others as required.
- Promote and maintain effective liaison with all
branches of the Hospice multidisciplinary team and to secure effective liaison
and collaboration with services outside the Hospice with key stakeholders.
- Take the lead in developing appropriate patient and
clinical information about the services in all relevant formats.
- Ensure that Lone Worker Systems for the Hospice are
implemented and clinical staff working in the community take adequate
precautions for their personal safety.
Education and Training
- Keep up-to-date with current legislation and
professional practice issues relevant to the role.
- Ensure
that appropriate induction, development and mentorship is in place for new
staff.
- Support the development of clinical skills and
knowledge within the Hospice Management Team as a whole, delivering training
sessions as required, as well as 1-1 support.
- Participate in the Hospice multi-professional
internal and external teaching programme, ensuring that self and others
perceive this as a vital aspect of their role and are trained to carry out
their duties and are supported to meet mandatory training requirements.
- Champion evidence based practice.
- In conjunction with Hospice colleagues, promote
research which is relevant to service needs.
- Ensure clinical placements with the community services
are effectively facilitated.
- Create a positive learning environment.
- Identify own and others learning needs and attend
training seminars, courses and study days as appropriate.
- Maintain own competence to meet professional, role
and registration responsibilities.
- Receiving ongoing support and clinical supervision.
Governance, Quality
& Risk
- Initiate and lead audits of managed services and review
to ensure outcomes are fed into relevant governance structure and findings are acted
upon.
- Carry out investigations into complaints and incidents
and report on these in accordance with relevant Hospice policies and guidelines
as well as local, regional and national standards and guidelines.
- Ensure
all governance, assurance, risk management and patient safety and quality of
care processes are in place.
- Contribute to the operational risk register and
advise on/support service business continuity in the event of significant
risks/issues.
- Develop an understanding of clinical governance
within the managed teams and ensure that feedback is provided in an open and
supportive way.
- Participate in forums for discussion of research and
audit programmes.
- Work with the Nurse Consultant to support the development
and implementation of development programmes.
- Foster an environment where research and audit is part
of practice within clinical services.
Grief and Loss- Support colleagues in managing loss, including
providing support for distressed patients, bereaved relatives, students and
staff.
- Support and educate colleagues in issues surrounding
tissue and organ donation.
- Support staff who are bereaved, referring them on to other
support if necessary.
General
Statements
The post holder will be
with and adhere to all Hospice (and relevant Trust) policies and procedures.
The post holder will work
within their professional code of conduct, e.g. NMC, HCPC, GMC, CIPD, AHP
codes.
The post holder may be
required to work in and from any Hospice or associated Company premises.
This job description is

expected to maintain the complete confidentiality of all material and
information to which he/she has access and process and in particular the
confidentiality of all personal data stored, in line with the requirements of

Acts) and professional bodies.
Any requests for clinical
information disclosure must be approved by Caldicott Guardian.
The post holder must, if
required to do so, process records or information in a fair and lawful way.
They must hold and use data only for the specified, registered purpose for
which it was obtained and disclose data only to authorised persons.
