Job summary
- To work as part of the multidisciplinary team to provide a high standard of individualised patient / family care and support.
- To be responsible for those patients allocated on a daily basis.
- To ensure that excellent standards of support and clinical practice and care are achieved.
- To manage a caseload of our higher acuity patients with daily virtual and potentially in-person reviews in their own homes.
- To work with wider Hospice team as well as system partners to support delivery of care and enable patients to remain in their preferred place of care.
Main duties of the job
Clinical Practice & Leadership
- Supporting those patients allocated from day to day in partnership with other members of the multidisciplinary team and wider system partners.
- Ability to work autonomously with designated patients alongside a vast degree of flexibility due to the unpredictable nature of the role.
Leadership & Management
- Supporting and reporting back to senior medical team, virtual ward colleagues, District Nurses, GPs.
- Supporting and facilitating the orientation and teaching of new and existing staff, colleagues and students.
Communication
Contributing positively to anti-discriminatory practice, actively promoting equality and diversity and inclusion from all you interact with.
Leading (as required) and participating in MDT, Virtual ward round, ongoing discussions and team handovers relating to the care of patients and their families/carers.
Quality
- Continually monitoring outcomes of care, taking effective action as required.
- Initiating clinical reviews of patient and family care.
Grief & Loss
- Supporting staff in managing loss in the community, providing support for distressed patients, bereaved relatives, students and staff.
Education & Training
- Identifying own and others learning needs, liaising with managers and attending training seminars, courses and study days as appropriate.

The Hospice is a local charity dedicated to providing free, high-quality, compassionate care and support for people with a terminal illnesses, their families, and their professional carers in the London Boroughs of Royal Greenwich and Bexley.
We care for over 2,500 people each year in the community; in their homes, care homes, our Hospice beds, Hospice-based clinics, local prisons and in hospital.
We receive only about one third of our funding from the NHS, which means that we depend on the generosity of our community to fund much of the care we provide for local people.
