Job summary
An exciting opportunity has arisen for a senior nurse to work across Weymouth and Portland to support development of services for Children and Young People, with a particular focus on inequalities. This role is ideally suited to a senior nurse or ACP, and experience of working with this population is essential. It is part time hours 16 hours per week. It is initially offered as a fixed term 12 month contract, but if successful then there is scope to become permanent. You will join a team including a lead GP for CYP and a Lead ACP for Inequalities - with strong administrative support.
Main duties of the job
You will be working for all of the Practices within the Weymouth and Portland PCN - The Bridges Medical Centre, Cross Road Surgery, Wyke Regis & Lanehouse Medical Practice, Royal Manor Healthcare, Weymouth Bay Medical Practice.
The overall aims of the role are to:
With support, identify the inequalities in health care provision and access to GP services for children and young people in the Weymouth and Portland PCN. Plan and carry out practical interventions to reduce or remove barriers leading to an increase in access to health care in a variety of ways which could be unique and creative.
