Job summary
Weymouth and Portland PCN is
looking for a Cancer Care Coordinator to join our existing friendly team of
care coordinators. The successful candidate will be based at a central
location, supporting all 6 surgeries in Weymouth and Portland.
You will need to be caring,
dedicated, reliable and person-focussed and enjoy working with a wide range of
people. You will have good written and verbal communication skills and strong
organisational and time management skills. We are looking for someone who is
highly motivated and proactive with a flexible attitude, keen to work and learn
as part of a team and committed to providing people, their families and carers
with high quality support.

Main duties of the job
Care coordinators play an
important role within a PCN to proactively identify and work with people,
including the frail/elderly, early diagnosis of cancer and end of life care
patients. You will provide coordination and navigation of care and support
across health and care services for patients with a diagnosis of cancer. The
individual will also work with the PCN team, supporting the establishment of
processes to improve early detection of cancer.
The post-holder will ensure
cancer patients receive appropriate information, onwards referrals and support
as needed. They will also support the practices within the PCN to improve their
services, including uptake of national cancer screening. They will also support
MDT meetings and manage a caseload of patients identified through these
meetings, working in conjunction with the Cancer Clinical Lead.
This is a
pivotal role and is required to support multidisciplinary teams and coordinate
care for patients with cancer.

- Work with individual practice teams and primary care professionals within the PCN to identify and manage a caseload of patients
- Work closely and in partnership with PCN colleagues including Social Prescribing Link Workers, Health Coaches and Clinical Pharmacist(s).
- Signpost team members, patients and carers to relevant services as appropriate
- Make referrals to the multi-disciplinary team as appropriate
- Central co-ordination role around proactive support and care planning for agreed patient cohorts (cancer specifically for this role)
- Support patient independence, promote healthcare and well-being including the use of digital apps to support their care
- Supporting patients and carers to utilise decision aids when appropriate
- Support patients to have shared decision making conversations
- Empower patients, this may include the use of validated PAM tools
- Facilitate smooth and planned discharge and handover between care settings across the health and social care system, including GP, acute, community, and be responsible for facilitating inter-agency communication and support
- Support people to take up training and employment, or to access appropriate benefits where eligible for example, through referral to social prescribing link workers;
- Assist people to access self-management education courses, peer support or interventions that support them to take more control of their health and wellbeing;
- Use healthcare technologies to optimise service delivery, access, and continuity of care for patients and carers
- Use Population Health Intelligence and Risk Stratification tools proactively to identify groups of patients for MDT discussions or to deliver health services
- May attend and present at MDT meetings or local schools
- May support the coordination and delivery of surgery MDT meetings
- May support in the setting up and administration of Group Consultations
