Job summary

We have an exciting role to join our innovative and publicly acclaimed Palliative Hub Team as a Clinical Nurse Specialist. This is a part time role working 30 hours per week, the role will primarily be performed from home and we can be flexible with the contract base being either Cambridge or Wisbech. This role can be performed from home and is for someone who can work across a 7-day period, providing 24 hours a day advice and support as part of a rota. It is a requirement of the role to visit the hospice at least on a monthly basis.

Job responsibilities

We have an exciting role to join our innovative and publicly acclaimed Palliative Hub Team as a Clinical Nurse Specialist. This is a part time role working 30 hours per week, the role will primarily be performed from home and we can be flexible with the contract base being either Cambridge or Wisbech. This role can be performed from home and is for someone who can work across a 7-day period, providing 24 hours a day advice and support as part of a rota. It is a requirement of the role to visit the hospice at least on a monthly basis.

Our Palliative Hub service provides specialist palliative and end of life care to any adult (age 18 or over) who is registered with a GP within the Cambridge and Peterborough Integrated Care Board via a telephone advice line and through NHS 111 service. We help end of life patients to receive care and support throughout the Cambridgeshire area. The enhanced services provide a single point of access (SPA) through a centralised hub for: patients, families, carers, health and social care professionals.

The aim of the telephone advice line is to support patients in the community to remain in their usual place of residence for as long as it is appropriate and safe to do so. Providing support for patients, carers, and healthcare professionals with the aim to avoid unnecessary or unwanted acute admissions.

A key element of this service is to appropriately clinically manage telephone calls from out-of-hour services, on-call doctors (HUC), the East of England Ambulance service, and other community services for patients, their families, and carers.

Please note, this is not a patient facing role.

A successful Clinical Nurse Specialist in this position will be expected to holistically assess, plan, implement and review therapeutic interventions of patients who contact the palliative Hub advice line by telephone, helping to avoid unnecessary hospital admissions.

Willingness to learn as a qualified non-medical/independent prescriber is essential, unless you already have the relevant qualification
