Job summary

The Therapy team at Arthur Rank Hospice Charity provides specialist palliative care to patients across our hospice services, including our in-patient unit, Specialist Palliative Care team and the Living Well Service. We have an opportunity for an Occupational Therapist to join this team in a full/time working hours. The working hours can be discussed during the interview.

Main duties of the job

The role will work closely with Physiotherapy colleagues to provide care, focussing on fatigue management, anxiety management and breathlessness management.

Successful candidates must be HCPC registered with a diploma / degree in Occupational Therapy, along with knowledge of, and experience of providing, palliative care to patients. Due to the nature of the role its a requirement to hold a UK driving license and have access to a vehicle for work purposes.

Interviews for this role will be held on the 17th and 18th September.

    Job description

    Job responsibilities

    You will be part of the Therapy Team which provides specialist palliative care to patients across the clinical areas of IPU, SPCHT, LWS.

    Job Purpose:

    To deliver a high standard of Occupational Therapy services to patients referred for Specialist Palliative Care. To plan and deliver those services in partnership with multi-disciplinary team colleagues on the in-patient unit, outpatients, living well service and with patients living in the community, to achieve/ maintain quality of life regardless of diagnosis and prognosis. To maintain associated records in accordance with HCPC and RCOT guidelines.

    To actively participate in improving the service and in collaboration with the line manager, support future developments. These will include involvement in clinical audit, education and research programmes within the organisation.

    Main Responsibilities:

    Clinical

    To provide holistic Occupational Therapy assessment and diagnosis using a rehabilitation model that includes the physical, psychological, social, emotional and spiritual aspects of the person referred for specialist palliative care, with diverse or complex needs and multiple pathologies, using professional clinical reasoning skills and evidence-based practice and to formulate appropriate treatment plans based on clinical reasoning

    To actively identify goals, plan suitable Occupational Therapy (OT) intervention, deliver and evaluate OT service provision, in partnership with patients, carers, multi-disciplinary team colleagues, and other professionals/ agencies, making appropriate referrals to others as required

    To communicate effectively with patients, families and carers especially where there are barriers to understanding, requiring empathy, reassurance and persuasive skills. Maintaining honesty and integrity in all situations.

    To communicate effectively with multidisciplinary team colleagues and other professionals/ agencies in the provision of care for patients with complex, emotional/psychological, spiritual, and physical needs and refer to appropriate disciplines. Attend relevant meetings as required.

    Where appropriate, contribute to the effective discharge of patients from the inpatient unit liaising with patient, family, colleagues, and members of the

    primary healthcare team

    To arrange and carry out timely access and home visits.

    To order appropriate equipment for safe discharge or to maintain safety in the patients home

    Participate in teaching, assessing and evaluation of OT students to graduate level within the clinical area of specialist palliative care.

    To actively contribute to the Education programme provided by Specialist

    Palliative Care Services

    Work flexibly across service areas to meet service needs.

    To act as a Moving and Handling link person for all clinical areas to facilitate Moving and Handling risk assessments and risk reduction measures.

    To be responsible for maintaining own competency to practice through CPD activities and to maintain a portfolio which reflects personal development. To participate in clinical supervision and in the staff appraisal scheme

    To undertake delegated responsibility for the assessment and evaluation of junior staff appraisals.

    To identify and address gaps in service delivery and working practices within specialist palliative care services, through independent observation or through joint working with others
