The Hospice at Home Team support people who wish to spend their last days in their own home. We provide symptom control and psychological and emotional support to not only the patient but also to their loved ones who are caring for them. We are looking for a committed registered nurse to join our team. With support from the Hospice at Home Sister, you would be responsible for planning and managing care for patients wishing to die at home.
Main duties of the job
What youll bring
To be successful in this role you will need to be a registered nurse with a passion for palliative care, have excellent communication skills, and a desire to work in the community. Community and/or palliative care experience would be an advantage.

a Registered Nurse you will deliver this by providing professional care and
support to our patients and families in their own homes, working alongside a
Hospice at Home Sister/Charge Nurse.

Key roles and responsibilities:
- Provide specialist nursing care to
patients in their own homes. Carry out
assessments, arrange care plans, monitor symptoms and adjust medication with
the guidance of the senior nurse on duty and working closely with HCAs and
Senior HCAs, provide personal care to patients.
- Liaise with other community
agencies to provide the best care.
- Provide emotional support to
families.
- Verify deaths and provide care
after death with the support of the Healthcare Assistants and Senior Healthcare
Assistants.
- Work across the 24/7 service
provision.
- Promote
a culture of continuous learning and development and wellbeing, help to create an environment
that is continually critically questioning practice and promoting learning.
- Identify your own learning and
development needs undertake continuous professional development and actively
engage in clinical supervision, maintaining an objective and innovative
practice.
- Provide
education both internally and externally. Provide mentoring and support to
junior members of the team and students.
- Play
an active part in team meetings and ensure your mandatory and other relevant
training is kept up to date.

Field worker the role could be carried out at a Rowcroft site or other
location subject to the varying needs of the role. Will spend planned time at Rowcroft sites or
other locations as the role requires which could include patient homes and
Rowcroft and non-Rowcroft locations to meet the needs of the service.
This will be fully discussed and agreed with the
postholder to meet individual and business needs.

important to the patient and those closest to them, working with other
