Job summary
Herts Health GP Federation are looking for Clinical Pharmacists to join our bank and support our GP practices across Hertsmere.
We have availability for additional shifts both during core hours and evenings and weekend working.
Main duties of the job
We would expect the pharmacist to work across the Network to achieve joint aims for all practices, as well as looking at individual work within each practice. Our Bank PCN Pharmacist roles cover evening and weekend appointments for medication reviews & queries and workflow management support for medication actions from letters.

Herts Health LTD is the GP Federation comprising 8 practices in the Hertsmere locality.
We have a board of four directors; each a partner of one of our member GP practices.
We work collaboratively with our practices and the PCNs in our locality to deliver services at scale.

Job responsibilities
Patient facing and remote consultations.
- The post holder will provide leadership on person centred medicines optimisation (including ensuring prescribers in the practice conserve antibiotics in line with local antimicrobial stewardship guidance) and quality improvement, whilst contributing to the quality and outcomes framework and enhanced services through structured medication reviews.
- The post holder will undertake Structured Medication Reviews (SMRs) and follow-up consultations in line with detailed guidance.
- Use appropriate clinical decision-making tools to support the delivery of SMRs and clearly record all SMRs within GPIT systems, as well as using appropriate clinical codes to signify the reasons for an SMR.
The post holder will proactively manage:
- Patients in care homes as per the Enhanced Health in Care Home specification.
- Patients with complex and problematic polypharmacy, specifically those on 10 or more medications.
- Patients who are being prescribed medicines that are commonly and consistently associated with medication errors.
- Patients with multiple long-term conditions and/or multiple co-morbidities in particular respiratory disease (e.g. asthma, COPD) and cardiovascular disease (e.g. Hypertension).
- Housebound, isolated patients and those with frailty particularly patients who have had recent admissions to hospital and/or falls.
- Patients who have received a comprehensive geriatric assessment as per the anticipatory care requirements.
- Patients with severe frailty and patients prescribed high numbers of addictive pain management medication.
The post holder will:
- Involve patients in decisions about prescribed medicines and support adherence as per NICE guidelines to reduce waste and promote self-care.
- Review the on-going need for each medicine, a review of monitoring needs and an opportunity to support patients with their medicines ensuring they get the best use of their medicines (i.e. medicines optimisation).
- Develop a tool box of resources e.g. Evidence base tools for drug selection and deprescribing.
- Make appropriate recommendations to fellow Pharmacists or GPs for medicine improvement.
Patient facing medicines support
- Provide patient facing and remote clinics for those with questions, queries and concerns about their medicines in the practice
Telephone medicines support
- Provide a telephone help line for patients with questions, queries and concerns about their medicines.
- Clinical review of condition from notes and/or patient via telephone
- Clear, concise documentation of review, any advice given and plan for ongoing treatment in electronic notes
- Prescribe medication to patients if requested where appropriate and to scope of prescribing qualification.
Medicine information to practice staff and patients
- Answers relevant medicine-related enquiries from GPs, other practice staff, other healthcare teams (e.g. community pharmacy) and patients with queries about medicines.
- Suggesting and recommending solutions.
- Providing follow up for patients to monitor the effect of any changes
Management of medicines at discharge from hospital
- To reconcile medicines following discharge from hospitals, intermediate care and into care homes, including identifying and rectifying unexplained changes and working with patients and community pharmacists to ensure patients receive the medicines they need post discharge.
Repeat prescribing
- Implement practice repeat prescribing and other prescription related policies.
- Manage the repeat prescribing reauthorisation process by reviewing patient requests for repeat prescriptions and reviewing medicines reaching review dates and flagging up those needing a review.
- Ensure patients have appropriate monitoring tests in place when required.
Signposting
- Ensure that patients are referred to the appropriate healthcare professional for the appropriate level of care within an appropriate period of time e.g. pathology results, common/minor ailments, acute conditions, long term condition reviews etc.
Medicines quality improvement
- Undertake clinical audits of prescribing in areas directed by the GPs, feedback the results and implement changes in conjunction with the practice team.
- Implement systems for required ongoing monitoring.
Medicines safety
- Implement and document changes that result from MHRA/CAS alerts, product withdrawal and other local and national guidance.
Risk stratification
- Utilise tools such as Eclipse Live for identification of cohorts of patients at high risk of hospital admissions or harm from medicines.
- Review at risk patients to reduce unplanned hospital admissions
Implementation of local and national guidelines and formulary recommendations
- Make recommendations to GPs for medicines that should be prescribed by hospital doctors (red drugs) or subject to shared care (amber drugs).
- Provide newsletters/bulletins/e-mails on important prescribing messages.
- Discuss and follow individual practice or local CCG protocols for DMARDs; controlled drugs; drugs of abuse; mental health drugs, antimicrobial prescribing and over the counter medications.
Management
- Demonstrate understanding of the process for effective resource utilisation
- Demonstrate understanding of, and conforms to, relevant standards of practice
- Demonstrates ability to identify and resolve risk management issues according to policy/protocol
- Follows professional and organisational policies/procedures relating to performance management

Collaborative Working Relationships
- Actively work toward developing and maintaining effective working relationships both within and outside the practice and locality
