We are looking to recruit to the post of
Clinical Pharmacist/Care
Home Pharmacist to work within our Primary Care Network (PCN) multidisciplinary
healthcare team. Southport and
Formby PCN is comprised of 14 practices throughout the Southport and Formby
area. We recognise the value that pharmacists can bring to our practices and
our patients, and we look forward to growing our PCN pharmacy team.
The postholder will work as part of a
multidisciplinary team, advising and supporting colleagues, in line with local
and national guidance, to promote high quality, safe and cost-effective care
for patients. They will become part of a group of PCN Clinical Pharmacists
working under the supervision of the PCN Clinical Pharmacy Team Manager.
The post holder will support care homes within the PCN with structured medication reviews. This
will involve working closely with our Enhanced Health in Care Homes team.
Although this role is advertised as a full time role, part time hours will be considered.

Main duties of the job
Working within GP Practices around the Southport and Formby area and our office in Southport Business Park, the post holder will take responsibility for areas of chronic disease management and undertake clinical medication reviews to proactively manage patients with complex polypharmacy. They will also provide clinical leadership on medicines optimisation and quality improvement and manage some aspects of the quality and outcomes framework and enhanced services.
The post holder will ensure that the practice integrates with community and hospital pharmacy to help utilise skill mix, improve patient outcomes, ensure better access to healthcare, and help manage workload. The role is pivotal to improving the quality of care and operational efficiencies, so requires motivation and passion to deliver an excellent service within general practice.
The post holder will
also support care homes within the Southport and Formby area with structured medication reviews. This
will involve working closely with our Enhanced Health in Care Homes team.
Successful candidates will be supported in their professional development through access to a GP clinical supervisor, other clinical pharmacists and enrolment onto a formal education pathway delivered through the Centre of Postgraduate Pharmacy Education (CPPE) (if not already completed). The post holder will be expected to develop their role to become an Independent Prescriber on completion of the CPPE Primary Care Education Pathway.
