Southport and Formby Health Ltd are a GP owned company created to enhance the delivery of healthcare services to people living in Southport and Formby.
Southport and Formby Health Ltd work closely with and are the principal employer for Southport and Formby Primary Care Network (PCN). Southport and Formby Primary Care Network is a group of 14 GP Practices located in the Southport and Formby area who work together with other local organisations to improve the health and health care for our local population of around 129,000 people.
We are looking for an individual who has people management experience and also understands the requirements of working within primary care services in relation to KPIs, financial budgets, operating procedures and governance standards.

The role is advertised as 30 hours, working Monday to Friday, however the applicant will be required to be flexible and be available to work at evenings and weekends within the 7-Day Service when required.
Main duties of the job
This is a dual role covering the 7-Day GP Service and projects within the GP Federation and both roles report into the Medical Director.
The 7-Day GP Service Manager role is responsible for the strategic leadership and direction, operational delivery, and direct management of the 7 Day GP Service which serve the population of Southport and Formby. The Service Manager plans and oversees the service and line manages the 7 Day GP Service Team Leader and Senior Administration staff, to provide an effective, safe and responsive service.
The Project Development Lead role will work with the Medical Director to develop plans for performance and service improvement in new and established project areas. It will involve providing day to day support for project activities which include project planning, monitoring and management, change control, quality, risk and issue monitoring and project reporting.
Key responsibilities for the roles above are outlined in the attached Job Description but may evolve in line with the innovative direction of Southport and Formby GP Federation.

7-Day Service Delivery:
- In conjunction with the Medical Director, be responsible for
the direct delivery of the Services.
- Provide strategic oversight of service operations to ensure
delivery of patient-centred care that meets KPIs for quality, safety, and
access. Drive continuous improvement through proactive performance management
and leadership.
- Contingency planning for pandemic measures and unforeseen
events.
- Lead the development of secure, efficient processes and
policies that support the full patient journey, from referral to
discharge.
- Communicate effectively and sensitively with patients, carers
and service users, colleagues and staff, respecting confidentiality and dignity
of every person.
- Develop and implement a robust audit framework to ensure
service standards are met and to evidence safety, effectiveness, and
responsiveness. Ensure all activities comply with organisational and statutory
requirements, including health and safety, infection control, quality
management, data protection, safeguarding, duty of candour, and general duty of
care.
- Membership of the Governance Committee, which monitors
compliance with CQC regulatory standards, and provide detailed reports as
required for each meeting.
- Identify and assess service risks, implementing effective
mitigation strategies. Maintain and regularly update the organisational risk
register, escalating significant concerns to the Governance Committee as

- Oversee patient enquiries and lead Learning Event and

with the 7-Day GP Team Leader.
- Be accountable for expenditure relating to the Services and understand
the impact of planned service change on the income and expenditure of the

experience of the Services and be responsive, working with staff to provide
continuous improvement and excellence.
- Participate in contract meetings with the commissioners to
