Job summary

Southport and Formby Health (SAFH) are a GP owned company created to enhance the delivery of healthcare services to people living in Southport and Formby.

Southport and Formby Health work collaboratively with Southport and Formby Primary Care Network (PCN). Southport and Formby Primary Care Network is a group of 15 GP Practices located in the Southport and Formby area who work together with other local organisations to improve the health and health care for our local population of around 129,000 people.

The role will support the Digital Clinical Hub at Southport and Formby Primary Care Network. The post will help to support GP practices remotely by performing key core functions such as:

  • Working to process clinical correspondence for all aligned Southport and Formby GP practices.
  • Utilising current models of practice such as workflows and templates for accurate recording of clinical information.

The individual must also be willing to undertake the appropriate training as set out by the Personalised Care Institute as part of the national framework for Care Coordinators. The individual must have experience of using clinical systems such as EMIS or be willing to undertake the necessary training, this is essential to the role.

This role is a new role within the PCN, and advertised as full-time, however part-time applications will be considered.

Main duties of the job

The job duties include the following;

  • To assist the PCN in developing processes that improve the way that clinical correspondence is coded, filed, and processed to benefit both practices and patients.
  • Support Practices with coding and filing all appropriate clinical correspondence who are aligned with the Digital Clinical Hub.
  • To aid in the direct care of patients by requesting investigations, arranging appointments, and improving the readability and understandability of the electronic health record.
  • Utilise current models of practice such as workflows and templates for accurate recording of clinical information and help develop appropriate reporting systems.
  • Accurately coding records using standardised SNOMED coding to improve data quality.
  • Engage in audit and analytics work to aid the digital clinical hub and PCN practices.
  • Assist member practices in digitising workflows.
  • Be part of a team providing this support, by working with colleagues to share skills, expertise, and knowledge to improve processes for all of our GP practices.
  • Provide administrative support to GP practices that request additional assistance regarding clinical correspondence or administrative requests, as agreed with the PCN.
  • Attend any relevant identified training that will assist in the development of the Digital Clinical Hub and your own personal development.
  • Build and develop relationships with team colleagues, wider PCN colleagues and with practices across the PCN.
