of administrative tasks to help signpost patients to the correct healthcare service

role will become an integral part of the practice team and will be supported by
the wider practice leadership team.
You must possess strong communication skills and be able to communicate effectively with clinical and non clinical colleagues both internally and externally.
The Care Coordinator will be part of the multidisciplinary team working within our Primary Care Network (PCN) to provide an all-encompassing approach to personalised care.
The role is advertised as full-time but part-time applications will be considered.

Main duties of the job
As a Care co-ordinator you will work closely with clinicians and the Lincoln House Surgery team acting as a central point of contact to ensure appropriate support is available to a caseload of patients. Helping patients with longstanding conditions manage their condition and access the service and support they require i.e. social prescribing link workers, health and wellbeing coaches and other professionals where appropriate.
The successful candidate must be caring, dedicated, reliable and person-focussed and enjoy working with a wide range of people. The individual must have experience of using
clinical systems such as EMIS and be competent in the use of Microsoft Office applications.
You will need to be highly motivated and proactive with a flexible attitude, keen to work and learn as part of a team and committed to providing patients, their families and carers with high quality support.

Southport and Formby Health Ltd is a local GP Federation, that specialises in the provision of primary care clinical and support services. Our main focus is supporting primary care services and helping to develop and grow the workforce of the Primary Care Networks (PCN) we serve.
Lincoln House Surgery is based within Southport and Formby PCN in a predominantly residential area, providing healthcare for approximately 2,000 patients.

There will be a
specific focus of the role to contribute significantly on the following key
themes (as well as being part of the reception team and the opening/closing rota for the surgery)

Manage Cancer Registers - Organise
cancer care reviews with lead Clinicians, ensure cancer care reviews are completed
and coded correctly and complete referrals to Cancer Navigators.

Management of Cancer safety-netting
templates.
-
Manage Gold Standard Framework (GSF)
registers Identify patients, manage register, organise and minute quarterly
GSF meetings and complete referrals as necessary.

Manage Learning Disability (LD)
registers - Arranging LD reviews with Practice Clinicians, ensure Action Plans are
completed and shared, manage Action Plan review meetings and use TeamNet LD
resources to comply with LQC requirements.

Manage Severe Mental Illness (SMI)
registers - Arrange blood tests and clinical reviews, ensure reviews are completed
and coded correctly using the correct templates, identify hard to reach
patients and discuss with Clinicians on how best to manage the reviews. Refer
to hard to reach service where appropriate.

Manage Long Term Conditions (LTC)
reviews in practice - Ensuring all patients eligible have review on time,
reviews are coded properly, and appropriate referrals are completed. Highlight
coding issues and performance at quarterly practice meetings.

Manage Housebound register Ensure
records are up to date and accurate.

Support recall systems for Local
Quality Contract (LQC).

Manage ICT MDT within the surgery. Create an appropriate referral process, arrange a system for Clinicians to dial
into meeting and ensuring actions are recorded and noted in clinical records.

The
post holder will assist with engagement with hard-to-reach populations and to
reduce health inequalities.

The
individual will be expected to engage and link in (where appropriate) to other roles within the PCN such as Social Prescribers, Health and Wellbeing Coaches
and other programmes of work such as Complex Lives.

closely with other PCN member practices to share good practice and learning
across the PCN.

other duties as may be reasonably required within the general terms of the job
