Job summary

We are looking to recruit a creative and motivated individual for the role of Digital Care Coordinator. The role will support the digital hub at Southport and Formby Primary Care Network (PCN).

The role will be integral to the digital transformation of our organisation and offers exciting opportunities in digital healthcare and innovation. The individual will work closely with senior management and the specialised care teams within the Primary Care Network to develop digital health services.

The role is advertised as being full-time however part-time applications will be considered.

Main duties of the job

The Digital Care Coordinator is a new post within our expanding digital department, focused on developing, managing, and enhancing digital health pathways. The role aims at identifying and supporting vulnerable patients, those with mental health problems, and those requiring lifestyle and weight management support.

Core Responsibilities:

Develop and Manage Digital Health Pathways: Develop, implement, and oversee digital health care pathways, ensuring they are patient-centric, efficient, and aligned with NHS values.

Patient Identification and Support: Identify and support vulnerable patient populations, including those with mental health challenges, through digital health platforms.

Lifestyle and Weight Management: Utilize digital tools and resources to support patients requiring lifestyle modifications and weight management assistance.

Collaboration: Collaborate closely with GP surgery staff to ensure digital health pathways are suitable, accessible, and beneficial for all patients.

Quality Improvement: Monitor and evaluate the effectiveness of digital health interventions, using data to drive continuous improvement and ensure alignment with NHS quality standards.

The individual must be enrolled in, undertaking, or qualified from appropriate training as set out by the Personalised Care Institute and will work with the Social Prescribing Link Workers or social prescribing service provider and Health and Wellbeing Coaches.

Southport and Formby Primary Care Network (PCN) is a group of 16 GP Practices working together with other local organisations to improve the health and social care of our local population of around 125,000 people.

Primary Care Networks are designed to provide proactive, coordinated care to their local populations, in different ways to match different peoples needs, with a strong focus on prevention and personalised care. This means supporting patients to make informed decisions about their own health and care and connecting them to a range of statutory and voluntary services to ensure they can access the care they need first time.

  • In support of collaborative working the post holder will form productive and supportive relationships with practice and PCN staff who have the skills, knowledge, and remit to contribute to this work. e.g., GP practice digital champions, practice IT leads, practice managers, social prescribers, pharmacists, medical secretaries etc
  • The post holder will assist the PCN member practices to engage with hard-to-reach populations and to reduce health inequalities.
  • The post holder will keep up to date with National/Local mental health and population health strategies.
  • Use the available population health intelligence tools to proactively identify and work with veterans, carers, pre-diabetics, and those requiring weight management support
  • Assist patients in accessing self-management education courses, peer support or interventions that support them to take more control of their health and well-being

  • Provide coordination and navigation for patients and their carers across available health and care services, working closely with social prescribing link workers, health and wellbeing coaches, and other appropriate primary care professionals
  • Source appropriate resources, training, and system examples from appropriate organisations such as the NHS Diabetes Prevention Programme (NHS DPP), NHS Prevention Digital weight management, local mental health services and local healthy lifestyle management services
  • Build relationships with community assets and develop a service directory of support for the patients within our PCN.
