Job summary

Lewis-Manning Hospice Care is an independent charity that offers holistic care to local people living with cancer and other life-limiting illnesses and support for their carers.

Would you like to join us? This important position will assess, plan, implement and evaluate holistic, individualised, high-quality care to patients and carers who attend the day hospice facilities both face to face and virtually. The role will support and deputise for the Clinical team lead and senior staff nurse and HCAs, as part of our multi-disciplinary team. The role will support volunteers in clinical areas and help to develop and maintain links with the community to provide seamless care for our patients. It is also expected that this role will contribute to further the development of our Anticipatory Care Model providing our Hospice at home service to patients in our community and our Rapid Home from hospital model reducing hospital re-admissions and providing care for patients in the last 2 weeks of life enabling them to die at home.

Main duties of the job

Reporting to the Clinical Team lead, this role will deliver care appropriately, facilitating colleagues from other disciplines as necessary, while providing relevant, accurate and timely information to patients and their families. You will complete telephone consultation calls and virtual sessions, taking open access calls from patients and carers providing advice as appropriate. You will also be responsible for facilitating effective discharge planning, in conjunction with inter-professional teams and outside agencies. This position will be responsible for the safe handling and administration of medicines in the Day Hospice, following strict supervisory and data recording procedures and liaising with GPs and consultants as appropriate. You will be responsible for ensuring adequate and accurate maintenance of patient documentation and records using System One in accordance with NMC guidelines and charity policy.

We are looking for someone with a first level RN qualification and at least one years experience, ideally with some experience in oncology or palliative care. You will already have experience of terminally ill and dying patients and caring and communicating with their families with a basic knowledge of palliative care and symptom control. You will have an awareness of the Gold Standards Framework and Mental Capacity Act and the NMC revalidation process.

This role requires you to travel to our satellite provisions across the county and undertake home visits.

Deliver care as planned and in keeping with the needs and wishes of patients and their families

Respond appropriately to clinical emergencies in the Day Hospice Facility.

Understand and analyse patient clinical observations and records, taking or instigating appropriate action as necessary

To provide appropriate information for patients and their families, which is relevant, accurate and timely and helps them to make informed decisions.

To take responsibility in overseeing medicines management in the Day Hospice. Facilitate and assist patients in the management of their own medicines, following strict procedures when supervising patients taking their medication. Liaise with patients own GP to confirm accurate medication list is being adhered to.

Be responsible for the safe handling and administration of drugs and the accurate recording of data relating to this in accordance with NMC and national guidelines and LMT Policy & Procedures

Be responsible for ensuring adequate and accurate patient documentation and records are maintained in accordance with NMC guidelines and LMT policy

Active in clinical governance to ensure high standards of care

Use research/evidence-based practice to ensure high quality, cost-effective care

Contribute to the further development of services for patients and their carers, which is needs-led and evidence-based

Assist the Team Leader and senior staff nurse in patient excursions outside the hospice

To be an effective part of the Nursing Team

To ensure the provision of a high level of specialist palliative care within a multidisciplinary approach, for patients and their family/carers.

To support the principles of team nursing, and oversee the work of junior staff

Manage the day to day running of day care, including our Satellite Day Hospices

Supervise and mentor all junior staff including students, bank nursing staff, junior registered and unqualified staff.

To performance review up to 3 Health Care Assistants and Complete junior staff 1-1s and appraisals

Ensure a safe environment for patients and staff

Manage the shift, planning and allocating tasks and care as appropriate in the absence of the senior staff nurse.

Support volunteers working in the clinical areas

Be familiar with the policies and procedures at LMHC and to work within these

Comply with the Health and Safety at Work Act and LMHC Policies and Procedures

Offer informal support to staff and volunteers concerning education and advice with palliative care issues

To be responsible for maintaining a personal awareness of professional developments in nursing in general but particularly in palliative care and to undertake relevant further education.

Attend team meetings and updates

To develop and maintain links with the community to provide seamless care for patients

To support other teams within the Organisation to promote an ethos of harmony within the hospice.

To act as a mentor/supervisor to Students and Health Care Assistants

Liaise and support the fund-raising team/activities
