Job Responsibilities
In the absence of the Clinical Team Leader or senior staff nurse, take responsibility of leading the morning MDT and co-ordinate the clinical day, including preparing the facility to receive patients. Acting as Duty Nurse and hold the duty phone, dealing with calls from patients, GPs, carers and outside agencies. Ensure that the Day Hospice area is clean as per infection control policy and tidy at the end of the day.
To be directly involved in patient care by regularly assessing and implementing appropriate individualised high-quality care, setting agreed care objectives with the patient. Evaluate progress at an agreed time to ensure objectives are met.
Ensure the care remains appropriate and facilitate colleagues in these processes
Deliver care as planned and in keeping with the needs and wishes of patients and their families
Respond appropriately to clinical emergencies in the Day Hospice Facility.
Understand and analyse patient clinical observations and records, taking or instigating appropriate action as necessary
To provide appropriate information for patients and their families, which is relevant, accurate and timely and helps them to make informed decisions.
To take responsibility in overseeing medicines management in the Day Hospice. Facilitate and assist patients in the management of their own medicines, following strict procedures when supervising patients taking their medication. Liaise with patients own GP to confirm accurate medication list is being adhered to.
Be responsible for the safe handling and administration of drugs and the accurate recording of data relating to this in accordance with NMC and national guidelines and LMT Policy & Procedures
Be responsible for ensuring adequate and accurate patient documentation and records are maintained in accordance with NMC guidelines and LMT policy
Complete telephone patient consultation calls, patient virtual sessions. Take open access calls from patients and carer, providing advice as appropriate
Facilitate effective discharge planning in conjunction with the inter-professional team and outside agencies
Undertake comprehensive patient assessments, in Day Hospice or in patients own homes, including End of Life conversations and Advanced Care Planning, ensuring policies, procedures and standards are applied
Promote good working relationships and collaborative working with the inter-professional team within the whole of the Organisation
Active in clinical governance to ensure high standards of care
Use research/evidence-based practice to ensure high quality, cost-effective care
Contribute to the further development of services for patients and their carers, which is needs-led and evidence-based
Be familiar with the LMHC clinical policies and procedures and work within these
To abide by LMHC policies on uniform code and infection control policies in general, including hand hygiene.
Assist the Team Leader and senior staff nurse in patient excursions outside the hospice
To be an effective part of the Nursing Team
To ensure the provision of a high level of specialist palliative care within a multidisciplinary approach, for patients and their family/carers.
To support the principles of team nursing, and oversee the work of junior staff
Manage the day to day running of day care, including our Satellite Day Hospices
Supervise and mentor all junior staff including students, bank nursing staff, junior registered and unqualified staff.
To performance review up to 3 Health Care Assistants and Complete junior staff 1-1s and appraisals
Ensure a safe environment for patients and staff
Manage the shift, planning and allocating tasks and care as appropriate in the absence of the senior staff nurse.
Support volunteers working in the clinical areas
Be familiar with the policies and procedures at LMHC and to work within these
Comply with the Health and Safety at Work Act and LMHC Policies and Procedures
Offer informal support to staff and volunteers concerning education and advice with palliative care issues
To be responsible for maintaining a personal awareness of professional developments in nursing in general but particularly in palliative care and to undertake relevant further education.
Attend team meetings and updates
To develop and maintain links with the community to provide seamless care for patients
To support other teams within the Organisation to promote an ethos of harmony within the hospice.
To act as a mentor/supervisor to Students and Health Care Assistants
Liaise and support the fund-raising team/activities
The above is not an exhaustive list of duties and you may be required to perform additional tasks within the organisation.

- Report (without delay) any accidents, incidents, near-miss events, risks, faults or defects
- Use all work equipment (medical devices, dangerous substances, machinery, transport, means of production, and safety equipment) in accordance with training and instructions provided
- Not use such items or equipment unless they have received appropriate information and training
- Have a good understanding of our MyConcern online safeguarding tool receive training and complete any of the above incidents immediately
Infection Control

Ensure high level of personal hygiene, hand hygiene in particular

Any matters of a confidential nature, including individual staff records must under no circumstances be divulged or made available to any unauthorised person(s).
Disclosures of confidential information or disclosures of any data of a personal nature can result in prosecution for an offence under the Data Protection Act 1998 and GDPR regulations 2018, or an action for civil damages under the same Act in addition to any disciplinary action taken by the organisation which might include dismissal. You should consult your line manager if you consider that there is a need to breach such confidentiality. You are advised that throughout your employment the organisation will collate information to support performance management.
