PCN Senior Development Manager (FT) based on-site.

You will be part of a team that works closely to support and learn from one another, and across PCNs.

  • You will be a qualified Project Manager with at least 2 years of experience.
  • You will be able to demonstrate delivery in the primary care sector.
  • You will understand the pressures faced by staff in GP practices.
  • You will be a team player with excellent communication skills.
  • You will have an eye for detail and a sound understanding of common project management processes.

Main duties of the job

We are looking for a dynamic and motivated person with experience of primary care and project management.

You will be used to working in a fast-paced changing environment whilst and using resources to create service models.

You will:

  • Ensure the delivery of key targets and a range of business, transformation, and improvement initiatives across the PCN providing leadership and supporting strategy.
  • Develop strong relationships and engage with a wide range of stakeholders across the PCN to ensure collaborative approaches to service development and delivery of the recommendations of the Fuller Stocktake.
  • Be responsible for managing agreed projects which focus on addressing inequalities and improving population health in the PCNs. The post holder will ensure that these are evidence based, data driven and directly address the needs of the local population

About us

The West London GP Federation (London Medical Associates) was established in 2014 and restructured in 2020 to a "Federation of PCNs"

The LMA Federation has developed a range of primary care services services at scale to support our PCNs and practices which include: a hub delivery of extended hours and frailty home visiting service, supporting ARRS roles in practices, and administrating the suite of out of hospital services delivered by GP practices in West London. The Federation covers five PCNs ranging in size from 31,000 to 102,000 population.

This role is part of a wider integration and transformation team across the five PCNs to share best practice and offer peer support and learning. You will be innovative in nature and willing to lead at a place level.

Transformation oversight and delivery

  • Support the Clinical Director to lead the overall planning and oversight of PCN development and transformation.
  • Lead and support the review and development of systems to monitor PCN development and maturity, and practice resilience and performance.
  • Interpret national and local health policy and guidance in areas of responsibility to inform the work of the PCN.
  • Update, maintain, organise, gather, and analyse information to predict/meet future organisational and team needs.
  • Develop relationships with current and potential partners to progress mutually beneficial initiatives and service developments.
  • Promote positive working relationships with other services and other organisations and agencies externally.

Project Management

  1. Systematic approach to project delivery and governance o Ensure that business cases, project initiation document (PIDs), project plans, and service specifications are produced to a high standard and that projects meet key milestones.
  2. Chair or participate in relevant project team meetings or working groups, providing expert advice and information, and coordinating stakeholders in the development and delivery of designated projects
  • Work with clinicians and service leads to identify service development and re-design opportunities to deliver efficiencies in care delivery and/ or improvements in patient experience for the PCN population.
  • Ensure that meaningful systems for involving service users, staff and the public in business and service development are established.
  • To horizon scan ensuring that all digital innovation related to primary care and general practice is reviewed and assessed against the needs of the PCN, leading on pilots of new digital technology where appropriate.
  • Maintain an up-to-date knowledge of and apply all new directives, health services guidelines, advance letters and any legislation that will impact upon service developments.

Primary Care Resilience and Developments

  • Undertake analysis of performance data and capacity and demand data to identify areas for service redesign and collaborative working.
  • Present complex, sensitive and highly contentious information and solutions to managers and teams in a manner that generates understanding and support, having analysed the evidence and options.
  • Where necessary work with practices who are underperforming to develop action plans based on the NWL Clinical Effectiveness model

Population Health

  • Support Population Health Management (PHM) & Clinical Effectiveness (CE) including the development and implementation of PHM and CE workstreams across practices and the PCN interfacing with colleagues in the, ICB, borough NHS team, inequalities and WSIC team.
  • Use appropriate data and population health tools to identify need in the PCN and to evaluate impact of key projects and interventions.
  • Promote and develop services in a way which demonstrates the impact population health outcomes.
