Seisdon Primary Care Network are
looking for a dynamic and enthusiastic paramedic to join our established team
working across all of the Practices in our Primary Care Network. We have a
close and expanding team of ARRS clinicians serving around 50,000 patients in
Perton, Codsall, Bilbrook, Featherstone, Wombourne and Claverley. Our practices
will provide the successful candidates a supportive environment to develop and
grow.
Main duties of the job
Undertake home visiting for housebound
patients and those patients residing in Care Homes
Assess patients presenting with acute illness
or for review of long term chronic diseases.
Assess and triage patients, including same day
triage and as appropriate provide definitive treatment or make necessary
referrals to other members of the primary care team.
Diagnose, plan, implement and evaluate
treatment/interventions and care for patients presenting with an
undifferentiated diagnosis.
Clinically examine and assess patient needs
from a physiological and psychological perspective, and plan clinical care
accordingly.
Assess, diagnosis, plan, implement and
evaluate interventions/treatments for patients with complex needs
Proactively identify, diagnose and manage
treatment plans for patients at risk of developing a long-term condition (as
appropriate).
Diagnose and manage both acute and chronic
conditions, integrating both drug- and non-drug-based treatment methods into a
management plan.
Advise patients on general healthcare and
promote self-management where appropriate, including signposting patients to
other community or voluntary services.
About us
Seisdon Primary Care Network have an exciting opportunity
for aParamedic/ACPto join ourestablished Primary
CareNetwork team supporting our practices in Wombourne, Claverley,
Perton, Bilbrook, Codsall and Featherstone.
Our PCN comprises 8 GP Practices and has a population of
approximately 50,000 patients.
As a Paramedic, you will work across all practices from
Monday to Friday. Occasional out of hours and weekend work may be required.
We are an enthusiastic, dynamic and friendly PCN who
constantly strive to improve patient pathways and health care outcomes.
Paramedics work autonomously within the community using
their enhanced clinical assessment and treatment skills, to provide first point
of contact for patients presenting with undifferentiated, undiagnosed problems
relating to minor illness or injury, abdominal pains, chest pains and
headaches. They are health professionals who have the capability to make sound
judgements in the absence of full information and to manage varying degrees of
risk when there are complex, competing or ambiguous information or uncertainty.

plans and lead certain community services (e.g. monitoring blood pressure and
diabetes risk of elderly patients living in sheltered housing).
Provide an alternative model to urgent and
same day GP home visit for the network and undertake clinical audits.
Communicate at all levels across organisations
ensuring that an effective, patient-centred service is delivered.
Communicate proactively and effectively with
all colleagues across the multi-disciplinary team, attending and contributing
to meetings as required.
Maintain accurate and contemporaneous health
records appropriate to the consultation, ensuring accurate completion of all
necessary documentation associated with patient health care and registration
with the practice
Other Duties
Work within the policies of the Practices, PCN
and GP First.
Promote client involvement in the management
of the service.
Participate in regular supervision sessions
and appraisals with your line manager. Attend training and development
activities as identified and participate in meetings as required.
Maintain administration systems and workspace
used in a clean and tidy condition.
Maintain a good working knowledge of Health
and Safety procedures and fire precautions, and operate the correct procedures
and participate in policy development and data collection where appropriate.
Work flexibly to meet the needs of patients
Undertake any other duties that are
commensurate with the post and within the post holders clinical competence.
