Job summary

This is an exciting, rare opportunity for an experienced social worker to join our innovative and dedicated Patient and Family Support Service.

We are looking for a qualified social worker, who has experience of working with Adults, Children and Young People, to provide support in our In-Patient Unit and in the community. The postholder will be the Adult Safeguard lead for St Clare Hospice and will jointly lead the Adult and Children and Young People Safeguard initiative.

To ensure we support as many patients, relatives, and friends as possible, the post holder will be required to work 11am to 7pm / 12pm to 8pm at least once per week.

Main duties of the job

To provide specialist palliative social work to hospice patients, their families, and carers#

To work as part of the multidisciplinary teams in the in-patient unit, therapy, and community team

To provide psychosocial social work as necessary to patients, family members and carers under hospice care

To assist in developing and growing the Hospices Children and Young People service to support those under 18 years of age who are living with someone with a life limiting or terminal condition or who have experienced a bereavement

To offer support, advice, and training to staff to promote psychological well-being

To present on relevant topics at the Hospices masterclass and study day programmes and participate in the Hospices wider education programme

To recruit and supervise student placement social workers

Job responsibilities

To be an integrated member of the multidisciplinary teams in the inpatient unit, therapy, and community team

To jointly manage the psychosocial team, including volunteers and students

To work in close co-operation with other members of the Patient and Family Support Team (PFST) to provide integrated specialist psychosocial palliative social work and bereavement support both internally and externally

To develop patient and family support in line with hospice strategy

To participate in the daily triage and assessment of all patients referred into the PFST ensuring the safety and appropriate response time of all referrals

To make and sustain links with external agencies and sources of social support relevant to the provision of specialist palliative care at St Clare Hospice

To jointly lead the Children and Young people safeguarding initiative and support in the design, delivery, and management of training for all hospice staff and volunteers as well as Mental Capacity Act (MCA) and Deprivation of Liberty Safeguarding (DoLS)

To contribute to the delivery of training to external agencies and organisations

To provide workplace support or supervision to other staff members where needed

To assist in the provision of teaching and support to nursing and other staff on issues such as social work skills and knowledge

To work closely with community engagement team members and volunteers to provide social supportive interventions, establish community links and develop the service

To manage own boundaries and raise any concerns around this

To participate in meetings, working groups and committees as necessary within the service

To maintain concise and accurate client / patient records on both IT systems and paper files
