Hospice has a brand new opportunity for an enthusiastic and passionate Nursing
Associate to join our team.
We are
looking for a highly motivated, proactive and compassionate Nursing Associate who
is qualified and on the NMC register with a Foundation Degree, to join our 8
bedded Inpatient Unit.
You will
work in partnership with our Registered Nurses and Healthcare Assistants to
provide the highest possible standards in palliative and end of life care.

Main duties of the job
Responsible
for delivering high quality patient centred compassionate careunder the supervision
of a Registered Nurse.
Work
as part of a multi-disciplinary team delivering care that focuses onthe holistic
needs of the individual.
Carry
out specific clinical and care tasks to a high standard.
Deliver
care independently in line with the individuals defined plan of care,within
the parameters of practice of the Nursing Associate role, accessingclinical
and care advice when needed.
Nursing
Associates will not routinely be expected to lead a shift but may berequired
to do so in exceptional circumstances where all other optionshave been
exhausted.

At
St Clare Hospice our vision is that every adult with a life limiting condition
in our local communities should have access to palliative care services
wherever and whenever they need them, so that they can make the most of every
moment, no matter how long they have left to live. Our many different
services provide patients, their families and carers, with physical, emotional,
social, psychological and spiritual support, at home, or here at the Hospice.
Our Inpatient Unit (IPU) is made up of eight private bedrooms and provides care
for adults over the age of 18, who are living with a terminal or life-limiting
illness. We offer specialist medical, personal and supportive care to those who
stay with us.
The IPU is staffed by a Multi Disciplinary
Team (MDT) that includes Registered Nurses, Nursing Assistants, Allied Health Professionals,
doctors, social workers administrators and volunteers. We work alongside the other clinical services
at St Clare including our Wellbeing Hub, Patient and Family Support Services, Hospice
at Home and community clinical nurse specialists.

To
be a safe practitioner and within the parameters of their competenceasdefined
by the NMC Code of Conduct for Nurses, Midwives and Standards of Proficiency
for Nursing Associates (NMC, 2018) and Standards of Proficiency for Nursing Associates
(2018)
To carry
out holistic needs assessments.
To
develop, update and monitor the individualised care plans ensuring they are
patient centred and support the individual patient needs.
To
deliver high quality palliative and nursing care interventions that are
responsive to the individual patients requirements.
To
communicate effectively with patients and families, ensuring they are actively
involved with their own care planning.
To
support patients and families through grief and loss and the life changing
consequences of a palliative diagnosis.
To
actively involve carers with practical nursing skills and enable them to
support the patient.
To
monitor patients for symptoms and administer prescribed medication, referring
for senior advice when appropriate.
To
undertake various nursing skills (once assessed as competent to do so) as
required such as insertion and removal of catheters etc.
To
ensure that all documentation in relation to patient care is accurate and
completed contemporaneously.
To
liaise with appropriate members of the multi- professional team (internal and
external to the organisation) in order to ensure co-ordination and continuity
of care on discharge and a smooth transition between different care settings.
To
work as part of the multi-professional team and to assist in the regular
evaluation and planning of care at all team meetings.
To
ensure that all equipment used within the IPU is clean and in good order in
accordance with infection control and health and safety policies and procedures.
To
access staff support and clinical supervision for personal professional
effectiveness and development.
To
work within other clinical areas within the hospice as required by the service
and if the need arises.
To
be practice within his/her scope of competency, seeking support and advice as
required.
To
be aware of his/her accountability and limitations in order to ensure safe
