Offer a telephone-based information gathering service which supports clinical decision making to establish mental health presentation, identifying risk and vulnerability for children, young people and families. Provide appropriate advice and information to enable professionals and families to access the right support at the right time
Main duties of the job
The main duties and responsibilities of the post will include:
Referral Management:
Respond to referrals in a timely and professional manner, following service policies and procedures.
Conduct concise information gathering by telephone to establish mental health presentation and impact on daily life.
Manage referrals and appointments, effectively utilising the patient management system
Support onward referrals to appropriate agencies.
Hold referral discussions with clinicians, providing concise information about mental health presentations..
Communication and Liaison
Maintain contact with professionals, children and young people, and parents/carers, providing accessible information with a clear rationale.
Attend internal and external meetings, providing accurate and up-to-date information.
Provide informed advice and signposting, demonstrating an understanding of available services and explaining rationale.
Record Keeping and Data Management
Accurately record contacts, demonstrating expertise in concise record keeping.
Follow information governance guidance, ensuring confidentiality of client information.
Contribute to data collection and reporting frameworks
Safeguarding and Risk Management
Identify risk, vulnerability, and safeguarding concerns effectively and take appropriate action.

Northpoint is a BACP-accredited service operating Service Centres for single point of access (SPA) and central point of contact (CPOC) services in West Yorkshire. Our Service Centres facilitate referrals and requests for support related to mental health and emotional well-being and ASD/ADHD requests for assessment. Our model is digitally enabled to provide an efficient and informed service user experience, connecting people to the right local resources and support, at the right time, in line with their individual goals.
In addition to our Service Centres, Northpoint provides school-based support and therapeutic interventions and operates as the lead provider in select local mental health partnerships in collaboration with Integrated Care Boards (ICBs) and NHS Trusts.
