Job summary
This role offers a telephone-based information gathering service which supports clinical decision making by establishing mental health presentation, strengths, risks and vulnerabilities. The post holder will provide appropriate advice and signposting information to enable professionals and families to access the right support at the right time.
Main duties of the job
The duties and responsibilities of the post will include the following:
Respond to referrals promptly and professionally, always following service and organisational policies and procedures
Conduct concise information gathering by telephone to establish mental health presentation and impact on daily life
Speak with professionals, children, young people, and parents/carers, offering information which is accessible and easy to understand
Manage referrals and appointments, making effective use of the patient management system
Keep accurate and concise records, showing care and attention to detail in all contacts
Identify risks, vulnerabilities, and safeguarding concerns, and take appropriate action when needed
Follow information governance guidance to protect the confidentiality of all client information
Provide clear advice and signposting, helping families and professionals understand available services and why they may be suitable
Support referrals onwards to the most appropriate agencies
Take part in referral discussions with clinicians, sharing concise information about mental health presentations
Attend internal and external meetings, contributing accurate and up-to-date information
Support data collection and reporting processes to help monitor and improve the service
About us
Northpoint are commissioned to provide part of the Open Minds Partnership (Calderdale CAMHS – Child and Adolescent Mental Health Service). The service includes a single point of access (known as the First Point of Contact - or FPoC), which is the single point of entry for all mental health referrals for children and young people, as well as referrals for Autism and ADHD assessment.
The role of the FPoC team is to help ensure that children and young people access the right support at the right time. The FPoC team also offers advice, guidance, and consultation to the children’s workforce to help them to support young people with emotional health difficulties.
