Job summary

We have full-time and part time posts available within our adult community nursing teams. All community nurses use EMIS as the electronic clinical record system and are provided with a work Ipad and Iphone. You will work as part of a local team, closely with the local acute hospital, social care, mental health teams, GPs, and Practice Nurses to ensure care is personal and centered around local population needs.

Main duties of the job

We are seeking a highly motivated Registered Nurse to work within a wider team of nurses and health care assistants. You will work under the direction of a District Nurse, to assess, implement and evaluate person centred care for a caseload of patients, undertaking venepuncture, wound care, catheterisation, syringe drivers, end of life care and symptom control. On achieving all relevant competencies, the post holder will be reviewed for a Band 6 Senior Community Nurse post.

Post holders will be available to work Monday to Sunday between 8 am and 8 pm and be able to travel to patients homes frequently throughout the day across the locality and across Guildford and Waverley.

The post holder will be a Registered Nurse in the community community nurse who works as part of a team to deliver nursing care and treatment autonomously as delegated by and under the direction of the District Nurse.

Job Purpose:

The post holder will visit adult patients in their own home who have an identified nursing need including those with a long-term condition, complex care needs and needing care at home and the end of life.

The post holder will use an electronic patient held record system, EMIS as a record of care and all our nurses are provided with a work Iphone and Ipad for mobile working.

KEY RESPONSIBILITIES:

Communication

Communicates effectively with patients, carers, and all members of the multidisciplinary team

Maintains accurate and legible documentation and clinical records in line with professional and organisational requirements this includes contemporary paper records and electronic clinical record system, EMIS

Attends and actively participates and contributes to team meetings and daily handover

Patient Care / Safety / Quality

Participates in the assessment and care planning of patient care under the direction of the District Nurse and Senior Community Nurse

Implements and evaluates care and treatment in agreement with the patient and as defined in the care plan

Escalates any deviation from the care plan or change in condition at hand over and to the District Nurse / Lead Nurse / GP accordingly

Prioritises daily work in line with clinical / patient need and escalates where there are issues with undertaking daily visits

Implements and works to ensure evidence and research-based care and best practice

Initiates health promotion activities with individuals, patients, and carers

Takes responsibility for a defined caseload in the absence of the senior community nurse and / or District Nurse

Participates in risk assessment and promotes the safety and well-being of patients, staff and any other persons and assists in ensuring a safe work environment

Management and Service Development

Engages in and is actively involved in the development and implementation of adult community nursing service developments and initiatives

Participates in clinical audit and the implementation of changes to ways of working / clinical practice from audit findings, recommendations, and action plans

Ensures that all complaints, untoward incidents, accidents, and areas of clinical risk are identified and reported to the District Nurse and on the electronic incident reporting system (DATIX) in accordance with policy

Monitors and orders stock levels and equipment in line with budget and lean / efficient principles

Responsible for the security, care and maintenance of equipment ensuring standards of infection control and safety are maintained Development of self and others

Keeps up to date with professional nursing and regulatory requirements, health care developments, clinical evidence, National Service Frameworks for practice, national and local policies, and guidelines

Responsible for completing all own mandatory training within the required timeframes

Accesses and participates in regular clinical supervision and peer review
