are looking for an enthusiastic, motivated and forward thinking nurse to join
our current friendly nursing team. We are a small practice of around 3600
patients, striving to provide excellent standards.
We
are looking for a candidate with previous practice nurse experience.
Full
or part time applicants will be considered.
Occasional
weekend work required.
Salary
based on previous experience.

Road Surgery are run by South Tyneside Health Collaboration - a federation of
South Tyneside GP Practices.
There
may be a need to work across our other sites based in South Tyneside.
The job is STHC Payscale E, which equates to £38,032 - £45,965 per annum

General nursing duties including:

Job responsibilities
To
be responsible for the implementation of processes for the effective management
of patients with long-term conditions using evidence-based practice including
care for elderly and housebound patients.
Working
as a senior member of the practice multidisciplinary team, the post holder will
ensure nursing services are delivered effectively to the entitled patient
population.
The
Practice Nurse will be responsible for a number of clinical areas such as
health promotion, chronic disease management, and mentoring other members of
staff. Actively supporting the practice management team in the reviewing and
delivery of clinical policy and procedure.
Albert

clinical supervision and an appropriate named individual in the organisation to
provide general advice and support on a day-to-day basis.
