South Tyneside Health Collaboration is seeking a compassionate and proactive Nurse Associate to join the Enhanced Primary Care Mental Health Service. This role focuses on delivering structured physical health checks and monitoring for individuals who are at risk of, or living with, health conditions impacted by mental health needs.
This includes but is not limited to people living with dementia, serious mental illness (SMI), neurodivergent individuals, and those identify with gender dysphoria, as well as others whose mental health needs may affect their physical wellbeing.
You will work within primary care settings as part of a multidisciplinary team, helping to reduce health inequalities, identify emerging risks early, and improve long-term outcomes through proactive monitoring and person-centred engagement.

Main duties of the job
To deliver comprehensive physical health checks in line with national and local
clinical guidance
Undertake venepuncture and facilitate appropriate blood testing for long-term
condition monitoring
Complete physical observations including BMI, blood pressure, pulse, and
relevant screening measures
Identify early indicators of physical health risk and escalate concerns
appropriately
Contribute to SMI health check delivery and dementia physical health
monitoring
Support individuals who may experience barriers to accessing routine
healthcare
Work collaboratively with GPs, pharmacists, mental health practitioners, and
wider MDT colleagues
Ensure accurate and contemporaneous clinical documentation
Provide accessible health education tailored to individual need
Uphold infection prevention, safeguarding, and patient safety standards at
all times

- Registered Nurse Associate with current NMC registration
- Evidence of ongoing CPD
- Venepuncture competency (or willingness to complete training)
Desirable
- Training in long-term condition monitoring
- Training relating to dementia, SMI, neurodivergence, or gender dysphoria
Knowledge and Skills
Essential
- Understanding of the relationship between mental and physical health
- Knowledge of long-term condition risk factors
- Competence in venepuncture and clinical observations
