PCN Clinical Pharmacist

Job summary

The post holder will provide primary support to general practice staff with regards to Structured Medication Reviews for priority groups, as stipulated by the PCN DES, aswell as other priority chosen pieces of work such as Lipid Optimisation and Drug Monitoring as required.

Main duties of the job

  • See (where appropriate) patients with single or multiple medical problems where medicine optimisation is required (e.g. COPD, asthma). Review the ongoing need for each medicine, a review of monitoring needs and an opportunity to support patients with their medicines taking ensuring they get the best use of their medicines (i.e. medicines optimisation). Make, and enact where appropriate, recommendations to clinicians for medicine improvement.
  • Undertake clinical structured medication reviews with patients. Make, and enact where appropriate, recommendations to clinicians on prescribing and monitoring
  • Deliver Structured Medication Reviews for the below cohorts of patients in line with DES specifications.
  • Provide support and supervision to allocated Pharmacy Technicians where appropriate.
  • Contribute pharmaceutical advice for the development and implementation of new services that have medicinal components (e.g. advice on treatment pathways and patient information leaflets).
  • Analyse, interpret, and present medicines data and audits to highlight issues and risks to support decision making.
  • Implement changes to medicines that result from MHRA alerts, product withdrawal and other local and national guidance
  • Work with the general practice team to ensure the practices in the PCN are compliant with CQC standards where medicines are involved.
  • Support public health campaigns. To provide specialist knowledge on all public health programmes available to the public.

Knowledge and Skills

Essential

  • Understanding of evidence-based practice
  • Proven ability to evaluate the safety and effectiveness of own clinical practice
  • Good understanding of Primary Care Structure and General Practice
  • Knowledge of national standards that inform practice (e.g., National Service Frameworks, NICE guidelines etc)
  • Excellent interpersonal, verbal and written communication skills
  • Time management and ability to prioritise workload.

Desirable

  • Understanding and knowledge of policy developments related to the delivery of primary care services including General Practice, the GMS/PMS contract, Clinical Governance, Quality & Outcomes Framework
  • Use of Microsoft Office (PowerPoint, Excel etc)

Qualifications

Essential

  • Registered with the General Pharmaceutical Council
  • Member of the Royal Pharmaceutical Society of Great Britain or UKCPA
  • Postgraduate Clinical Qualification (MSc or Diploma)

Desirable

  • Teaching / Mentoring Experience and / or qualification
  • Hold or be working towards an Independent Prescribing Qualification

Experience

  • Minimum 2 years' experience as a qualified Pharmacist, demonstrated within a Practice portfolio
  • Has experience and an awareness of common acute and long-term conditions that are likely to be seen in General Practice
  • Recognises priorities when problem-solving and identifies deviations from normal pattern and is able to refer to GPs when appropriate
  • Able to follow legal, ethical, professional and organisational policies/procedure and codes of conduct.
  • Involves patients in decisions about prescribed medicines and supporting adherence as per NICE guidelines
  • Evidence of appropriate continuing professional development activity to maintain up-to-date knowledge
  • Proven ability to evaluate the safety and effectiveness of their own clinical practice.
  • Experience of patient consultation
  • Experience of Structured Medication Reviews
  • Experience of working with GPs
  • Experience of clinical pharmacy

  • 2 years' post-registration experience in Primary Care setting
  • Experience of conducting service audits and use of performance indicators
