STHC is a GP Federation, set up in 2017, to bring together GP practices to work collaboratively, delivering services at scale across the borough of South Tyneside and is formed of all 20 of the Practices in South Tyneside.

Main duties of the job

We are seeking a skilled and compassionate Physiotherapist to join our team to deliver specialist assessment and short-term primary care interventions for patients experiencing dizziness and balance disorders. The postholder will work across a variety of settings, including community clinics, patients homes, and care homes. They will also take a lead role in providing education and support to care home and community staff to promote falls prevention, safe mobility, and effective management of residents and patients with dizziness or balance issues.

This role combines clinical expertise in vestibular rehabilitation with a proactive approach to education and partnership working across community and care home environments.

Job description

Job responsibilities

Clinical Duties:

Conduct comprehensive assessments for patients presenting with dizziness, imbalance, or vestibular dysfunction.

Develop and implement individualized treatment plans, including vestibular rehabilitation, balance retraining, and mobility interventions.

Run dedicated Dizziness Clinics and provide home or community-based support for patients requiring vestibular or balance rehabilitation.

Collaborate with multidisciplinary teams (e.g., audiology, ENT, occupational therapy, nursing) to ensure coordinated care.

Support patients in care homes and in the wider community to maintain independence, reduce falls risk, and improve quality of life.

Monitor progress, review outcomes, and adjust treatment plans as required.

Maintain accurate and timely clinical records in accordance with professional and organizational standards.

Education and Training:

Deliver education sessions and workshops to care home staff on topics such as dizziness management, falls prevention, safe mobility support, and posture/movement strategies.

Provide practical advice and demonstrations to improve staff confidence and resident safety.

Support the development of training materials and resources for care home partners.

Contribute to the wider staff education programme within the service, promoting evidence-based practice in balance and vestibular rehabilitation.

Professional and Service Development:

Participate in service evaluation and quality improvement initiatives related to dizziness management and care home support.

Stay up to date with current research and best practice in vestibular rehabilitation and geriatric physiotherapy.

Provide mentorship and guidance to junior staff or students as appropriate.

Qualifications

Essential

  • Master's Level Qualification
  • BSc in Physiotherapy
  • HCPC Registration
  • Experience being a first point of contact for Clinical Issues
  • Experience in Community and Care Home Settings
  • Ability to work within competencies, referring to relevant departments/agencies where required
  • Understanding and appreciation of the role of PCNs in the NHS structure
  • Proven team membership skills
  • Ability to work autonomously within scope of practice
  • Excellent communication skills
  • Ability to work within NICE Guidelines and local pathways effectively
  • Knowledge of falls prevention strategies and frailty management
  • Understanding and appreciation of the wider Frailty agenda
  • Able to work with initiative but seek guidance and help where appropriate
  • Willingness to work flexibly
