Advanced Nurse Practitioner

Job summary

We are seeking an experienced and compassionate Advanced Nurse Practitioner (ANP) to join our Enhanced Primary Care Mental Health Service. As a vital member of our integrated multidisciplinary team, you will help provide holistic, coordinated, and person-centred care that improves outcomes and enhances quality of life. In this role, you will deliver personalised support for individuals living with long-term health conditions (LTCs) who are also managing a mental health condition.

You will play a key role in improving health outcomes, reducing health inequalities, and empowering patients to manage their conditions through shared decision-making, physical health monitoring, and tailored health interventions. You will contribute to a multidisciplinary team by fostering a culture of reciprocal learning. This will involve equipping colleagues from mental health backgrounds with new skills while also drawing on their expertise. Through this two-way knowledge transfer, the team will build shared understanding, strengthen collective capability, and drive more integrated and effective ways of working.

Main duties of the job

The ANP will work autonomously and as part of a team to support early identification of physical health issues, provide ongoing management of co-existing physical and mental health conditions, and ensure equitable access to healthcare for this vulnerable population. The post holder must be a qualified independent prescriber and will utilise their clinical skills to diagnose, manage, and prescribe as appropriate. The ANP will be clinically responsible to prescribe in the absence of a GP.

Job responsibilities

Clinical Practice:

Conduct physical health assessments and screenings for patients with long-term mental health conditions, including cardiovascular risk assessments, diabetes checks, respiratory assessments, and metabolic monitoring.

Undertake annual physical health checks for patients receiving shared care (e.g., antipsychotic monitoring in partnership on behalf of primary care).

Interpret clinical findings and initiate appropriate treatment plans or referrals.

Prescribe medications safely and appropriately, in line with local and national guidelines.

Manage and review physical health issues in collaboration with mental health and primary care teams.

Promote lifestyle advice and health education (e.g., smoking cessation, diet, physical activity).

Maintain accurate and up-to-date clinical documentation in line with local and national policies.

Leadership and Service Development:

Contribute to service planning and quality improvement initiatives focused on integrating physical and mental healthcare.

Support junior staff and contribute to the clinical supervision and training of colleagues where appropriate.

Liaise effectively with GPs, community teams, pharmacy, and other healthcare professionals to ensure continuity of care.

Governance and Professional Practice:

Adhere to clinical governance frameworks including risk management, incident reporting, and audit.

Maintain own professional development in line with NMC/HCPC registration and revalidation requirements.

Follow all local and national policies relevant to the ACP role, prescribing, and mental health services.
