Job summary
South
Tyneside Health Collaboration have established and embedded a primary care cancer
care coordination service for all patients in South Tyneside who are diagnosed
with cancer. We are looking to increase our existing team.
Main duties of the job
The
Care Coordinator will work with the network in supporting them with the NHS
England Early Cancer Diagnosis DES. A large part of your role will involve
supporting patients in the early stages of their cancer diagnosis through
treatment and living with cancer. Other work may involve supporting patients
with a suspicion of cancer on a cancer pathway. The role is key to supporting
patients after they have been diagnosed with Cancer and helping them to
understand the implications of their diagnosis and what the next steps are, matching
the support that is available locally to help patients during this worrying time.

STHC manages two of its own general practices, one in the beautiful coastal village of Whitburn and another, a developing training practice in central Jarrow, with a combined list size circa 8,500.

Successful
candidates will work with patients to complete non-clinical elements of cancer
care reviews and help patients understand why cancer screening is key for early
diagnosis and support them whilst awaiting diagnostic tests for possible cancer
and investigate those patients who fail to respond or attend for this. You will
act as the patients coordinator between hospitals, community and other services
and organisations.

Essential
- GCSE grade A to C
- Previous admin and clerical experience within the NHS or similar field
- Evidence of on-going personal development
Experience

- Experience of administrative duties
- Three years experience within NHS
- Knowledge and experience of using a range of databases, or patient information systems.
- Knowledge of primary care EMIS System
- Understanding of current issues facing the NHS
Desirable
- Knowledge of cancer standards
- Knowledge of medical terminology, anatomy, and physiology
- Previous experience of MDT co-ordination or working in cancer services
Skills and Attributes
