Job summary

To undertake clinical medication reviews for patients with chronic diseases and to proactively manage people with complex polypharmacy, especially the elderly, people in care homes, those with multiple long-term conditions. Provide expertise in the use of medicines and person-centred medicines optimisation across the network, including taking a central role in the clinical aspects of shared care protocols and liaison with specialist pharmacists to reduce inappropriate use of high-risk medicines Support the integration of general practice with wider healthcare teams (including community and hospital pharmacy) to help improve patient outcomes, reduce waste, and promote self-care, provide better access to healthcare, and manage general practice workload.

Main duties of the job

Knowledge and experience of the NHS

At least two years post clinical pharmacist qualification working experience

Ability to work across organisations to achieve shared aims for all partner practices.

Maintain appropriate documentation within the work portfolio of work underway with practices.

Support communication processes for all the work in hand.

Communicate effectively the progress of work in hand across all key stakeholders.

Qualifications

Essential

  • Registered with the General Pharmaceutical Council as a Pharmacist in the UK

Desirable

  • Postgraduate Diploma or equivalent (or be working towards one) - desirable
  • Independent prescribing qualification (or to work towards this in the future) - desirable

Experience

Essential

  • A minimum of 2 years post-registration clinical experience
  • Understanding of working effectively within multidisciplinary groups
  • Demonstrable experience in clinical pharmacy practice covering a broad range of specialities
  • Recognises the roles of other colleagues within the organisation and their role to patient care
  • Demonstrates use of appropriate communication to gain the cooperation of relevant stakeholders (including patients, senior and peer colleagues, and other professionals, other NHS/private organisations e.g. ICSs)
  • Demonstrates ability to work as a member of a team
  • Is able to recognise personal limitations and refer to more appropriate colleague(s) when necessary
  • Demonstrable experience of co-ordinating and leading on clinical audit.
