Job summary
To undertake clinical medication reviews for patients with chronic
diseases and to proactively manage people with complex polypharmacy, especially
the elderly, people in care homes, those with multiple long-term conditions.
Provide expertise in the use of medicines and person-centred medicines
optimisation across the network, including taking a central role in the
clinical aspects of shared care protocols and liaison with specialist
pharmacists to reduce inappropriate use of high-risk medicines Support the
integration of general practice with wider healthcare teams (including
community and hospital pharmacy) to help improve patient outcomes, reduce
waste, and promote self-care, provide better access to healthcare, and manage
general practice workload.

- Registered with the General Pharmaceutical Council as a Pharmacist in the UK
Desirable
- Postgraduate Diploma or equivalent (or be working towards one) - desirable

Experience

- A minimum of 2 years post-registration clinical experience
- Understanding of working effectively within multidisciplinary groups
- Demonstrable experience in clinical pharmacy practice covering a broad range of specialities
- Recognises the roles of other colleagues within the organisation and their role to patient care
- Demonstrates use of appropriate communication to gain the cooperation of relevant stakeholders (including patients, senior and peer colleagues, and other professionals, other NHS/private organisations e.g. ICSs)
- Demonstrates ability to work as a member of a team
