looking to recruit a motivated and experienced Pharmacist to work four days a
week across our Bromleag Care Practice (2 days) and Anticoagulation
Service (2 days). This is an excellent opportunity to be part of a

Care Practice:Bromleag Care Practice are looking to recruit a dedicated and proactive
Pharmacist to join our team two days a week. This role is integral to our
multi-disciplinary team (MDT) approach, enhancing the quality of care for our
residents through collaborative, patient-centred medication management and
clinical support.
The base
location for this role is:

Anticoagulation Service: KPFH Anticoagulation
Hub and clinics across various locations in Bromley and Lewisham
Bromleag Care
Practice:
Beckenham Clinic, including visits to Bromley Borough Care Homes, Nursing Homes
and Residential Housing.
Main duties of the job
Assist running of Community anticoagulation clinics
across Bromley/Lewisham
Work closely with service
team to assist achievement
of short/long term objectives
Facilitate anticoagulant prescriptions & ensure patients receive correct
anticoagulation medication
Promote safe, effective & economic use of anticoagulant drugs
within community & train healthcare staff in
anticoagulant management
Provide professional advice & support to care homes on safe prescribing, handling & administration of medicines. This
includes support of development & implementation of medicines
management processes & policies
Lead on medicines aspects relating to the care of
patients in care homes to optimise medicines, reduce medicines-related
risks & improve adherence to taking medicines, carrying out
medication assessments & reviews
Medicines management with face to face dealings with
patients, care home staff, relatives
Undertake prescribing for patients within own competency
according to needs of the service
Resolve day-to-day medicines & prescription issues with
healthcare professionals
Help patients & care home staff to manage the patients
conditions
Lead practice repeat prescription services & deal with
queries from reception staff & patients
Present to Patients & relatives any changes to
prescribing in line with NICE, best practice or in response to
prescribing alerts
Advising patients/carers of the adverse side effect of
medication and potential interaction with other medication/treatments

Job responsibilities
Anticoagulation Service:
Manage and support patient receiving anticoagulant therapy in the community, according to national standards of practice.
Support multi-disciplinary team of staff within the community involved in supporting patients on anticoagulation therapies including DOACs, enoxaparin and VKAs.
Help to promote standardisation in initiation and maintenance of anticoagulant therapy.
Regularly review patients on warfarin (including at the outset) with regard to indication, duration, compliance, target INR, and clinical safety and liaise/refer to appropriate clinicians and/or Consultant Haematologists for medical opinion.
Assess and identify the correct regime for patients outside the therapeutic range and refer to an appropriate health professional when necessary.
Screen patients referral for DOAC initiations
Provide support in a clinic, or by telephone, for prescribing, dose adjustment and close monitoring of patients on anticoagulant treatment following agreed protocols and patient group directions
Provide specialist advice to individual patients and to other healthcare professionals on the dose of anticoagulants in accordance with agreed protocols
Assess patients condition according to INR and counsel accordingly, taking into account the relevant clinical details.
Identify and follow up patients who fail to attend for INR checks.
Support in the management of patients requiring vitamin K.
Provide anticoagulant services in line with SELICB, NICE, NPSA and other relevant guidelines.
Support the management of patients on LMWH
Participate in the domiciliary visiting service for patients on anticoagulant therapy, either in person, or by supporting ancillary staff by telephone when they are visiting the patient at home
Bromleag Care Practice:
Rationalise repeat prescriptions lists to avoid waste and duplication
Provide care-home based reviews to those residents with LTCs or complex health needs, including drug monitoring and risk versus benefit.
Provide on-call pharmacist cover for the practice as per duty rota
Contribute to the management of the prescribing budget through the promotion of safe, cost effective rational prescribing for all residents in care homes
Contribute to minimising the harms associated with the use of medicines, for example by de-prescribing
Link to local and national care home forums to support embedding best practice.
Support the education of care home staff and other relevant bodies regarding medicines management.

Youll use your clinical and communication skills to carry out medication reviews, support prescribing decisions, and collaborate with GPs, patients, families, and ICB colleagues to drive safe, effective, and person-centred care.
The BGPA Anticoagulation Service is led by Clinical Pharmacists.
Their responsibilities include:
