Job summary
An exciting opportunity has arisen to work in conjunction with the appointed Bexley clinical lead, Bexley CCPL for Frailty & End of Life Care, and SEL palliative and end of life care programme (PEoLC) to complete a project focusing on Universal Care Plans (UPCs) in Bexley.This project aims to undertake an assessment of the baseline usage of UCPs and quality audit of their content. This assessment will inform recommendations for improvement and will guide the preparations for the planned expansion of the UCP functionality in order to maximise their impact for the benefit of Bexley residents.
Admin and/or care co-ordination support is needed to ensure the success of the project, including supporting with data collection and quality audit. Data analysis and visualisation will support the appointed clinical lead to make recommendations for quality improvements
Main duties of the job
Key Responsibilities
- Collaborate with the CCPL & appointed clinical lead to establish detailed data set requirements and project plan.
- Work alongside the appointed clinical lead to obtain & collate the agreed dataset, with particular focus on the qualitative data set.
- Maintain & store records of data collected in an appropriately secure manner.
- Produce tables/graphs/other visual aids to effectively communicate data to others, as part of reporting & educational activities
- Provide monthly update on project progress, including interim findings, to CCPL & SEL PEoLC senior project manager.
- Produce a full report of the projects findings upon completion & join CCPL to present these findings at the appropriate borough & ICB level meetings and educational events.
- Utilise contracted hours flexibly to support needs of the project.
