Job summary

The role of the Anticoagulation Pharmacist is to work within the BGPA Community Anticoagulation Service and managing patients on warfarin, as well as clinically screening incoming referrals and practising as a Pharmacist Independent Prescriber and conducting Initiation appointments and prescribing suitable Anticoagulants. Pharmacist is expected to work according to a Standard Operating Procedures (SOP) and PGDs.

This is a part time role, for 1 day a week (days can be flexible), predominantly in the Lewisham area, but candidate may also be required to cover clinics in Bromley area.

Main duties of the job

  • To assist in the running of the Community anticoagulation clinics

  • To work in close liaison with the anticoagulant dosing and monitoring team to assist in the achievement of short and long term objectives of the stroke prevention programme.

  • Use of INR STARCDSS software

  • To clinically assess referrals sent into community anticoagulation clinics

  • To facilitate anticoagulant prescriptions and ensure patients receive the correct anticoagulation medication

  • To promote the safe, effective and economic use of anticoagulant drugs within the community and to train other healthcare staff in anticoagulant management.

  • To be involved in specified project or group work.

Job description

Job responsibilities

  • To support the anticoagulant monitoring and dosing service to manage patient receiving anticoagulant therapy both in the community according to national standards of practice.

  • To support multi-disciplinary team of staff within the Community involved in supporting patients on therapeutic anticoagulation therapies including DOACs, enoxaparin and VKAs.

  • To help promote standardisation in initiation and maintenance of anticoagulant therapy.

  • To regularly review patients on warfarin (including at the outset) with regard to indication, duration, compliance, target INR, and clinical safety and liaise/refer to appropriate clinicians and/or Consultant Haematologists for medical opinion.

  • To assess and identify the correct regime for patients outside the therapeutic range and refer to an appropriate health professional when necessary.

  • To provide support in a clinic or by telephone for prescribing, dose adjustment and close monitoring of patients on anticoagulant treatment following agreed protocols, patient group directions or as an independent prescriber.

  • Provide specialist advice to individual patients and to other healthcare professionals on the dose of anticoagulants in accordance with agreed protocols

  • Assess patients condition according to INR and counsel accordingly, taking into account the relevant clinical details.

  • To identify and follow up patients who fail to attend for INR checks.

  • Support in the management of patients requiring vitamin K.

  • To interview / counsel patients with unstable or difficult anticoagulant control in a clinic setting, supported by telephone communication.

  • Provide education and training on anticoagulation therapy and monitoring to pharmacy staff and other healthcare workers.

  • Provide anticoagulant services in line with NICE, NPSA and other relevant guidelines.

  • Assist in thromboprophylaxis management.

  • Support in management of patients on LMWH

  • To participate in the domiciliary visiting service for patients on anticoagulant therapy either in person or by supporting ancillary staff by telephone when they are visiting the patient at hom

  • To assist in the running of the Community anticoagulation clinics

  • To work in close liaison with the anticoagulant dosing and monitoring team to assist in the achievement of short and long term objectives
