Job Summary

To work alongside the management team in assisting with the running of the Bromley GP Alliance services.

The post holder is an experienced clinician who, acting within their professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care. They will demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills, for patients within the general practice. The post holder will demonstrate critical thinking in the clinical decision-making process. They will work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership as required.

Main duties of the job

Duties and Responsibilities

Independently prioritise, organise and manage a highly complex patient caseload. Formulate differential diagnoses and specialist clinical decisions following highly specialist assessment; provide highly specialist, evidence-based intervention

Act as patient advocate in issues concerning their care.

Practice in compliance with their respective code of professional conduct and within their scope of practice, being responsible and accountable for their decisions, actions and omissions at this level of practice.

Demonstrate a critical understanding of their broadened level of responsibility and autonomy and the limits of own competence and professional scope of practice, including when working with complexity, risk, uncertainty and incomplete information.

Work in partnership with individuals, families and carers, using a range of assessment methods as appropriate (e.g. of history-taking; holistic assessment; identifying risk factors; mental health assessments; requesting, undertaking and/or interpreting diagnostic tests; and conducting health needs assessments)

Demonstrate effective communication skills, supporting people in making decisions, planning care or seeking to make positive changes.

Work collaboratively with an appropriate range of multi-agency and inter-professional resources, developing, maintaining and evaluating links to manage risk and issues across organisations and settings.

  • To deal with all telephone calls related to the services, ensuring callers are communicated with in a sensitive and effective manner and problems are dealt with promptly and efficiently.
  • Handle confidential patient material in a sensitive and discrete fashion in compliance with Bromley GP Alliance policy and procedure.
  • To be able to influence, motivate and involve individuals and teams to reach necessary performance target expectations.
  • Liaise with other departments and members of the Bromley GP Alliance and other organisations to ensure the smooth running of the office and to assisting the functioning of the team.

Post holders must maintain the confidentiality of information about patients, staff and other health service business in accordance with the Data Protection Act of 1998.
