Job Summary
To work alongside the management team in
assisting with the running of the Bromley GP Alliance services.
The post holder is an experienced clinician who, acting within
their professional boundaries, will provide care for the presenting patient
from initial history taking, clinical assessment, diagnosis, treatment and
evaluation of care. They will demonstrate safe, clinical decision-making and
expert care, including assessment and diagnostic skills, for patients within
the general practice. The post holder will demonstrate critical thinking in the
clinical decision-making process. They will work collaboratively with the
general practice team to meet the needs of patients, supporting the delivery of
policy and procedures, and providing nurse leadership as required.
Main duties of the job

Responsibilities
Independently prioritise, organise and manage a highly complex
patient caseload. Formulate differential diagnoses and specialist clinical
decisions following highly specialist assessment; provide highly specialist,
evidence-based intervention
Act as patient advocate in issues concerning their care.
Practice in compliance with their respective code of professional
conduct and within their scope of practice, being responsible and accountable
for their decisions, actions and omissions at this level of practice.
Demonstrate a critical understanding of their broadened level of
responsibility and autonomy and the limits of own competence and professional
scope of practice, including when working with complexity, risk, uncertainty
and incomplete information.
Work in partnership with individuals, families and carers, using a
range of assessment methods as appropriate (e.g. of history-taking; holistic
assessment; identifying risk factors; mental health assessments; requesting,
undertaking and/or interpreting diagnostic tests; and conducting health needs
assessments)
Demonstrate effective communication skills, supporting people in
making decisions, planning care or seeking to make positive changes.
Work collaboratively with an appropriate range of multi-agency and
inter-professional resources, developing, maintaining and evaluating links to
manage risk and issues across organisations and settings.

BGPA is a federation of all 43 Bromley practices working
collaboratively to enhance the health and wellbeing of Bromley residents,
covering over 350,000 patients, 100% of the Bromley population.

Communication
