Ward Clerk & Discharge Liaison (Band 3)

Job summary

The postholder will provide comprehensive clerical, administrative, and information technology support to medical, nursing, and other staff within the IPU. In addition, the role will act as the key discharge liaison, ensuring effective communication with Continuing Healthcare (CHC) teams, management of multidisciplinary team (MDT) documentation, and completion of all discharge-related paperwork

Main duties of the job

Provide an efficient and welcoming reception service for patients, families, and visitors. Support the smooth running of the ward through accurate and timely administrative tasks. Manage patient admissions and discharges, ensuring records and documentation are complete and up to date. Liaise with Continuing Healthcare (CHC) teams, GPs, and community services to coordinate patient discharges. Maintain and update patient information on the electronic record system (EMIS). Handle telephone and email enquiries promptly and professionally. Arrange transport for patient transfers, appointments, and discharges. Ensure ward paperwork, forms, and supplies are well organised and replenished. Communicate effectively with the multidisciplinary team to support efficient patient flow. Uphold confidentiality, accuracy, and compliance with organisational policies at all times.

Job responsibilities

Generic Ward Clerk Duties Welcome patients and visitors onto the ward, informing the Sister or Nurse in Charge of any queries. Ensure efficient administration for patient admissions, including checking, printing, and recording information on EMIS (patient record system) and in the admissions book, and preparing necessary paperwork packs. Carry out all administration associated with patient discharges, including updating records and ensuring correspondence is sent to GPs and Nursing Homes as appropriate. Handle administration following the death of a patient, including contact with GPs and Funeral Directors, updating records, and completing all paperwork. Take telephone calls for the unit and advice line, ensuring messages are accurately taken and passed on promptly. Keep main reception staff updated regarding patient admissions and discharges throughout the day. Arrange transport for hospital appointments, discharges, transfers, and relatives visits. Maintain adequate stocks of all IPU paperwork, care plans, and forms. Deal with post for the IPU and ensure key information and communications are printed and displayed. Keep the IPU diary updated and check it regularly to ensure required tasks are carried out. Work closely with the unit coordinator and perform delegated tasks related to the safe running of the IPU. Undertake any other clerical duties as designated by the Ward Manager/Nurse-in-Charge/Nurse Led Bed. Ensure all clerical details comply with departmental and Trust policies, procedures, and guidelines. Participate in the annual IPR process, ensuring all agreed objectives are completed. Liaise regularly with all members of the ward team, offering admin and clerical support freely. Discharge Liaison Duties Act as the primary point of contact for all discharge-related communications with CHC, ensuring timely updates and responses. Manage and maintain MDT documentation around discharge, ensuring all relevant information is accurately recorded and accessible to the team. Ensure the completion and submission of all discharge-related paperwork, including IPDANT forms, CHC funding applications, and CHC fast track referrals. Coordinate with GPs, Nursing Homes, and other external agencies to facilitate smooth patient transitions. Update patient records and ensure all necessary correspondence is sent to relevant parties.
