Job summary

Are you passionate about person-centred care and community wellbeing? We are seeking a dedicated and compassionate Community Wellbeing Lead to guide our Day Services multi-disciplinary team in delivering evidence-based care to individuals living with life-limiting conditions. In this vital role, you will champion a holistic approach that empowers patients through enablement and self-management, helping them to achieve their personal goals and maximise their wellbeing. Youll also play a key part in the development of our community hubs and lead the expansion of the services we offer in the community.

Main duties of the job

Lead the design, implementation, and delivery of group-based wellbeing activities, including the Living Well Programme, informal drop-ins, community-based services Work as a senior member of the Day Service and Therapy multi-disciplinary team, delivering evidence-based, compassionate, and person-centred care. Foster community engagement with a focus on improving health equity and access for underserved populations. Oversee day-to-day operational management of the Day service, including line management of a small team of staff and volunteers. Collaborate with internal clinical teams, community stakeholders, volunteers, and service users to promote inclusion, autonomy, and dignity throughout the palliative care journey.
