Job summary

We are expanding ourHospice at Home Teamand are looking for Band 5 Registered Nurses or Health Care Professionalsto join us.

Our service works across the Bristol, South Gloucestershire and North Somerset.

This role is mainly night shifts working with individual patients in the last weeks of life in their own homes.If you can work one or more nights a week you are welcome to apply.

The night shift runs from 21:15 07:15 and we can offera range of working hours, between 10 and 30 hours per week.

Main duties of the job

We are a friendly, inspiring, and supportive team, committed to providing high quality end of life care.

The role involves:

  • Working closely with our various professionals to support the physical and psychological needs of patients nearing end of life at home.
  • Assessing and monitoring changes in patients
  • Administering medication in response to symptom control.
  • Verification of expected death.
  • Excellent communication skills with the ability to liaise with our patients, their families, and various professionals.
  • Excellent organisational skills.
  • Adaptable and positive to the challenges of working overnight
  • Employee referral scheme - earn up to £500 for referring a friend to join the Hospice

You must have a UK driving licence and access to a vehicle to use at work.

The Hospice at Home Registered Nurse works independently in patients homes as part of the wider Hospice and community teams. They provide high-quality, evidence-based nursing care to patients in the last four weeks of life and support to their families/friends at the most challenging time. They are responsible for - the assessment, planning and delivery of care underpinned by evidenced based practice. working actively towards achieving the clinical competencies needed to meet the needs of patients under our care. Actively leading, guiding, and supervise junior staff, students and volunteers

Care and Quality To assess, plan, implement and evaluate individualised patient care in collaboration with the patient and their carers and the multi-disciplinary team. Ensure that patient privacy and dignity is maintained at all times, with special regard to issues of confidentiality. To deliver all care in a caring and compassionate manner, treating patients and colleagues with dignity and respect. To administer medications as prescribed and monitor and report on their effect. Support all patients and their families, respecting their beliefs and personal circumstances. Work within SPH Mental Capacity Act and Deprivation of Liberty Safeguards Policy to ensure you safeguard the interests of all patients. Carry out verification of expected death following St Peters Hospice guidelines. Support bereaved families after the patients death, informing them of the after-death processes that need to occur. Work within own scope of practice, taking professional accountability for clinical competence. To contribute to practice improvement and clinical audit, implementing any recommended changes, so ensuring evidence based, quality specialist palliative care is being delivered to patients, carers and their families. To contribute to maintaining Health and Safety standards and Risk Assessment within Hospice at Home. Use resources economically and efficiently. Prioritises own workload and manages own time to ensure priorities are met and quality is not compromised. To report accidents, incidents, and complaints as per policy, participating in investigations as required.
