Job summary
Mid Furness PCN are seeking an Enhanced Practice Nurse-Care Homes to join our growing team.
The successful candidate will be part of a dynamic and forward-thinking PCN,
Clinical leadership will be provided by the PCN Clinical Director and Clinical Leads.
We are looking for individuals who will lead clinical care home rounds & coordinate the work of healthcare professionals and non-clinical staff in the care of patients registered at GP practices within the PCN population.
The role includes responsibility to work with the practices to support and ensure compliance with the reporting requirements of the PCN Directed Enhanced Services (DES).

Evaluates and analyses clinical problems using their clinical knowledge, seeking out and applying relevant evidence, enhanced techniques, interventions, and equipment to make clinical decisions.
Delivers enhanced clinical care in the context of continual change, challenging environments, different models of care delivery, innovation and rapidly evolving technologies using analysis and their underpinning knowledge to manage complex interventions.
Facilitate and support the development and delivery of innovative training and education to staff, to improve competence and confidence, in line with best practice and guidance related to area of expertise.
Contribute to the delivery of local policy, procedures and protocols for the PCN to improve identification, assessment, ongoing support for patients and their family carers.
The Enhanced Practice Nurse will work closely with the Care Coordinators, GPs and practice teams to manage a caseload of patients, acting as a central point of contact to ensure appropriate support is made available to them and their carers; supporting them to understand and manage their condition and ensuring their changing needs are addressed.
Duties will vary from time to time under the direction of the clinical director and network management leads, in agreement with the post holder, dependent on current and evolving practice workload and staffing levels.
