Job summary

We are looking for a dedicated professional who shares our values and passion about delivering high quality, joined-up clinical management of patients both in clinic and in their own homes. The role would be suitable for a range of existing skill levels, from a Paramedic Practitioner with no experience in general practice, to an Advanced Clinical Practitioner Paramedic.

Main duties of the job

The aim of the role is to autonomously assess and treat patients with a range of conditions, seeing patients without prior contact with their GP. You will be confident in formulating rapid and accurate treatment plans and provide education to patients and service users, promoting self-care and empower them to make informed choices about their treatment

Job description

Job responsibilities

  • Provide a first point of contact within the practice for patients presenting with undifferentiated, undiagnosed problems, making use of skills in history taking, physical examination, problem-solving and clinical decision-making to establish a diagnosis and management plan.
  • To provide assessment, treatment and diagnosis at point of first contact by attending to patients in a variety of clinical or non-clinical settings according to patients needs.
  • Advise patients on general healthcare and promote self-management where appropriate, including signposting patients to other community or voluntary services, PCN services and to self-care.
  • Support the delivery of anticipatory care and end of life care plans and lead certain community services (e.g. monitoring blood pressure and diabetes risk of elderly patients living in sheltered housing).
  • Communicate at all levels across PCNs and other organisations, ensuring effective, patient-centred service
  • Communicate proactively and effectively with all colleagues across the multidisciplinary team, attending and contributing to meetings as required and accepting referrals and referring to specialist services where appropriate.
  • Develop relationships and collaborative working approach across the PCN supporting the integration of pathways in primary care.

  • Take an active part in achievement of contractual markers such as QOF and IIF.
  • Support the acute home visiting of patients from the 3 practices that make up Kendal PCN
  • Undertake advanced assessment of house bound patients, resulting in an end to end episode of care through treatment planning and prescribing (as required and appropriate within the scope of clinical competence and practice)
  • Gain insight into coordination of care of patient and feedback directly to practices to ensure excellence in integrated service delivery across the neighbourhood
  • Develop clinical practice, taking responsibility for your level of experience and qualification, actively seeking support and education when required.
  • Critically evaluate current research and demonstrate an understanding of the application of national and local guidelines and legislation relating to health and social care and their impact on service provision.
  • Implement and participate in vaccination programmes as appropriate.
  • Be responsible for and lead others in maintaining stock of equipment and materials, advising the correct procedures relating to the safe and competent use, issue and retrieval.
  • Be accountable for decisions and actions supported by a professional culture of peer networking/review and engagement in evidence-based practice.
