Job summary
We are looking for a highly motivated and committed Nurse Associate to join our Primary Care Network (PCN) and practice teams. In this key role, you will support the delivery of high quality primary care services and work closely with our multidisciplinary clinical team.
As a Nurse Associate, you will contribute to excellent patient care, help enhance the patient experience, and play an important part in the ongoing success and development of the PCN.
Main duties of the job
As a Nurse Associate within our PCN, you will:
- Deliver safe, evidence based, patient centred care across the lifespan
- Support clinical teams with assessment, planning, implementation and evaluation of care plans
- Conduct routine health checks, administer medications, and assist with minor procedures
- Perform diagnostic tests and assist with interpreting clinical information
- Provide health education and contribute to disease prevention initiatives
- Maintain accurate patient records and uphold data protection standards
- Collaborate with GPs, nurses, AHPs, and wider multidisciplinary teams
- Support referrals and ensure timely access to specialist services
- Participate in quality improvement and service development activities
- Engage in training and development to continually enhance your skills
