Job summary

We are looking for a motivated and compassionate Social Prescriber to join Worcester City Primary Care Network (PCN), a collaboration of 10 practices supporting over 127,000 patients. This is an exciting opportunity to make a real difference in peoples lives by helping them take control of their health and wellbeing.

As a Social Prescriber, you will work closely with patients, families, carers, and community groups to connect individuals with the right support at the right time. From tackling loneliness and poor housing to improving diet, physical activity and access to local services, you will play a vital role in addressing the wider factors that impact health.

Youll be part of our Wellbeing Services Team and the wider PCN network, building strong partnerships with healthcare colleagues, community organisations and statutory services. Every day will be varied and rewarding, as you co-produce personalised support plans, empower people to build resilience, and reduce health inequalities across Worcester City.

Main duties of the job

As a Social Prescriber, you will:

  • Work directly with people to understand their personal circumstances and connect them with the right mix of support.

  • Link individuals to community groups, local services, or practical advice on issues such as housing, finances, and employment.

  • Encourage healthier lifestyle choices through activities such as exercise, arts, or nature-based programmes.

  • Build trusting relationships and co-produce personalised support plans that help people take control of their wellbeing and live more independently.

  • Collaborate with GP practices, the PCN team, voluntary sector partners, and statutory services to provide joined-up support.

key responsibilities support people by connecting them to community based activities groups and services that meet their practical social and emotional needs including specialist advice services arts and culture physical activity and nature and green based activities provide personalised support to individuals their families and carers to take control of their wellbeing live independently and improve their health outcomes helping people identify the wider issues which impact on their health and wellbeing such as debt poor housing unemployment loneliness and caring responsibilities co produce personalised plans with the patient to improve health and wellbeing introducing or reconnecting people to community groups and statutory services manage and prioritise caseload in accordance with needs priorities and urgent support required by individuals refer people back to other health professionals and agencies where appropriate work within local communities and develop maintain good working relationships with the vcse sector local community groups and organisations to work within appropriate safeguarding procedures and practices to build effective relationships with key staff in gp practices attending relevant meetings and becoming part of the wider network team to work in partnership with other local agencies to raise awareness of social prescribing and to promote the service widely and its benefits to use appropriate systems for data capture case management feedback and reporting and adhering to data protection legislation and data sharing agreements to work as part of the wellbeing services team as well as the wider pcn sharing best practice and maintaining strong working relationships
