Job summary

Vale of Evesham PCN is dedicated to improving patient care and tackling health inequalities. We're looking for enthusiastic Care Co-ordinators to play a key role in providing support and ensuring residents receive the care they need.

Main duties of the job

Co-ordinate care for residents of care homes, ensuring their needs are addressed.

Develop and manage personalised care and support plans.

Work closely with GP practices and the Primary Care Network (PCN).

Support patients and their families in understanding and managing their health and wellbeing.

Collaborate with healthcare professionals to ensure seamless and integrated support.

  • Ability to work on own initiative and manage time effectively.
  • Knowledge of healthcare systems, safeguarding, and confidentiality.

  • Health and social care qualification.
  • Experience in a multidisciplinary team.
  • Familiarity with issues related to dementia and complex health needs.
  • Use of IT systems including EMIS and Microsoft Office.
