Job summary

We are looking for a dedicated Care Co-ordinator to join our team. You will play a crucial role in coordinating care for residents of care homes, helping them and their families understand and manage their personal care and support plans. This position involves working closely with GP practices to provide seamless, patient-centered care.

Main duties of the job

Identify patients needing care packages and coordinate their support

Develop and maintain personalized care plans in collaboration with patients, carers, and healthcare professionals

Assist patients in managing appointments and accessing necessary support services

Provide information and guidance to help patients make informed decisions about their care

Coordinate care across various health and social care services

Maintain accurate records and ensure timely updates to care plans
