Job summary

We are seeking a compassionate and proactive Social Prescriber to join our team. As a Social Prescriber, you will support patients by connecting them to community groups, healthcare, and statutory services for both practical and emotional support. Your role is crucial in helping patients make informed health choices, build resilience, and address factors impacting their health such as social isolation, poor housing, and physical inactivity.

Main duties of the job

Connect individuals to community-based activities and services that address their practical, social, and emotional needs.

Provide personalised support to help patients manage their wellbeing, live independently, and improve health outcomes.

Create tailored plans with patients to improve their health and wellbeing, linking them to appropriate community and statutory services.

Manage and prioritise caseloads based on individual needs and urgency.

Work within safeguarding procedures and build effective relationships with GP practices and local community groups.

Promote social prescribing and its benefits across the community and local agencies.

Use appropriate systems for data capture, case management, feedback, and reporting, adhering to data protection legislation.
