Job summary

Overview- To assist in the provision of an effective administrative and signposting service for Your Health in the community, to support the clinical functions of the service. Work in conjunction with colleagues, service users and stakeholders to provide an efficient clerical support service. To ensure that a high standard of customer care is provided at all points of contact and ensure this is done so using tact, discretion and sensitivity. Main duties of the job

Providing an efficient and professional administrative/signposting service: Registering patients to GP Practices both paper applications and on-line applications IT including researching suitable services the patient can be directed to Supporting patients with booking further appointments at their GP Practice Amending and sending out clinical documents (e.g. letters / referrals) Ensuring appropriate care records and files are available to clinicians (e.g. for inpatient and outpatient appointments and for ad hoc requests) Accurately inputting confidential data using a variety of SWHC IT systems in a timely way. Booking / utilising interpreters for patients as required Providing flexible administrative and reception cover as required to meet the needs of the services. Liaise with staff, service users and stakeholders Provide excellent telephone skills Accurately record telephone messages and other enquiries and refer as appropriate Provide effective communication and problem solving both face to face and via telephone

- Providing non-clinical advice and information to patients

- Collaborating as part of an integrated team

- Keeping up-to-date with National and Trust legislation, guidelines, policies, etc.

- Participating in the effective introduction of new systems and initiatives for service improvement

- Responsibility for Financial and Physical Resources:

- Having a personal duty of care for all equipment and resources

- Maintaining stock control of stationery/clinical supplies

- Logging end-of-day reports and patient case studies

- Supporting data gathering for monthly reporting

- Traveling around Worcestershire to attend clinics as directed by Team Leader
