Job summary

As the PCN Nursing Team Lead, you will play a vital role in supporting patients within our community to enhance their health outcomes. You will lead a team of nurses, nurse associates, and healthcare assistants, focusing on addressing health inequalities and working with diverse populations. This is an exciting opportunity to contribute to the improvement of healthcare services in Worcester City.

Main duties of the job

Key Duties:

  • Lead and support the nursing team to provide appointments for the local population.
  • Collaborate with the PCN Management team to plan services and enhance capacity across the city.
  • Champion efforts to reduce health inequalities in line with NHS contracts.
  • Provide guidance and leadership, ensuring adherence to clinical standards and governance.

  • A good knowledge of, and experience in, general practice, primary and community care
  • An interest in leadership within primary care and the wider health and social care system
  • Understanding of the emerging Primary Care Network arrangements and the implications for general practice
  • A good working knowledge and interest in new models to strengthen and improve primary care and ideas on how to implement these for the development of locally
  • An understanding of the key priorities within the PCN Network Contract DES, Core20Plus5 and NHS Long-Term Plan
  • An understanding of population health and population health management
  • Knowledge of local health issues within own PCN
  • Experience of working with a wide range of stakeholders

Qualifications

  • NMC Registered Nurse

Desirable

  • Currently working in a practice or PCN
