St Nicholas Hospice Care Community Nursing Team comprises
Hospice Specialist Nurses, Senior Registered Nurses and Hospice Care Assistants
all working together to provide excellent palliative care across the West
Suffolk and Thetford area.
Main duties of the job
Our Hospice Care Assistants within the community team support
patients, families, and carers to ensure patients can be looked after in their
own home supporting them by providing education to carers, social and emotional
support. They work closely with West Suffolk Hospitals Early Intervention Team
(EIT) and Marie Curie to try and keep patients from unnecessarily going into
the hospital.

standard of care to all patients and
providing formal and informal education to families and their significant
others on how to support someone with complex palliative care needs. The
STNH HCA is required to work both independently and as part of a team providing
safe individualised holistic palliative care, under the support and guidance of
registered healthcare professionals, guided by the relevant STNH protocols and
procedures.

- Competent in manual handling equipment e.g. hoists. Requirement to carry out manual handling training.
- Effective and sensitive communicator.

- Knowledge and experience in end of life palliative care.
