Social Prescriber - Gypsy Roma Traveller Service

Job summary

Proposed salary: Band 5.1 to 5.12 on the ABC pay scale, which is equivalent to £28,888.23- £35,918.88 per annum dependent on experience (pro rata)

Hours of work: 1 or 2 days per week (flexible hours/day) on a Zero Hours Contract basis

Base: Horley Health Hub, with outreach delivery across Surrey and Sussex

Main duties of the job

This role combines social prescribing, health promotion, and smoking cessation support specifically for Gypsy, Roma and Traveller communities across Surrey.

The post holder will:

Build trusting relationships with GRT individuals and families

Provide personalised support to improve wellbeing using Make every Contact Count (MECC)

Deliver smoking cessation advice, behavioural support and followup

Support access to primary care, screening, vaccinations and wider services by signposting and referring into appropriate services

Work collaboratively with ABCs Health Inclusion Teams, VCSE partners and local authorities

Social Prescribing and Personalised Support

  • Provide onetoone support, giving people time to tell their story and identify what matters to me.
  • Coproduce simple personalised wellbeing plans based on strengths, priorities and goals.
  • Help individuals address wider determinants of health such as housing, finances, social isolation, caring responsibilities and access to services.
  • Support people to register with GP practices and navigate primary and community care.
  • Make supported introductions to community groups, statutory services and VCSE partners.
  • Maintain a small caseload, prioritising according to need and urgency.

Smoking Cessation Support

  • Deliver one to one and group-based smoking cessation interventions on GRT sites.
  • Offer behavioural counselling, motivational support and relapse prevention strategies.
  • Educate individuals on risks of smoking, available treatments and coping strategies.
  • Support access to nicotine replacement therapy or other cessation options under clinical guidance.
  • Apply MECC principles during all interactions.

Outreach & Community Engagement

  • Build strong, trusting relationships with GRT communities through regular site visits.
  • Work collaboratively with local VCSE organisations, Traveller Education Services, community groups and statutory partners.
  • Promote health awareness including vaccinations, screening, mental wellbeing and selfcare.
  • Identify unmet needs and escalate gaps in provision to the Equity Manager & Clinical Lead.

  • Deliver care that is culturally sensitive, trauma informed and person centred.
  • Work with interpreters, support workers and advocates where needed.
  • Escalate safeguarding concerns in line with ABC and statutory procedures.
